Relevance Among Urinary Glycosaminoglycans,Heparanase Of Peripheral Blood Leukocyte And Serum Elastase In Nephrotic Syndrome

Objective: Glycosaminoglycans(GAGs) are the major component of the glomerular basement membrane.They are very important to keep the normal permeability of glomerular filtration barrier. Catheptic enzyme,for exampale, heparanase(Hpa) and elastase(EA)can split GAGs.But the Relevance among GAGsn Hpa amd EA remains unknown.The purpose of this study was to explore the relevance in the Steroid Responsive and Simple Nephrotic Syndrome(SRSNS).Methods:(1)Fifty-five children with SRSNS were selected,including the climax stage group (n=20),the convalescence stage group(n=20),the catabasis group(n=15).The 21 nephritic nephrisis, 19 purpuric nephritis and 15 healthy children were served as the control.(2)Using the improved Whiteman process detected the urinary glycosaminoglycans.Human elastase activities in plasma were determined by the amount of 4-nitroaniline released per unit time. Immunohistochemistry and image analysis method were imployed to detect the expression of heparanase of peripheral blood leukocyte.Results :1.All groups of Nephrotic Syndrome (NS) exhibite higher levers of GAGs than the healthy group(p<0.05).The climax stage group ofSRSNS was the highest (4.1051± 1.17220 ) of all( (P<0.001) .In contrast with the healthy group,there was significant difference between the climax stage group and the convalescence stage group (2.5666±0.68258) (p<0.001),but no diference to the catabasis group ( 1.6588±0. 31031 ) (p>0.05).The difference among the climax stage group -, the nephritic nephrisis(3.4126±0.61524) and the purpuric nephritis (3.3269±0.59168) is not statistic significant (p>0.05 ) .However, the climax stage group is higher than the nephritic nephrisis and the purpuric nephritis.2. All groups of NS exhibite higher levers of Hpa in peripheral blood leukocyte than the healthy group(p<0.05).The climax stage group of SRSNS was the highest (IOD: 54786.2137±28714.06712) of all( (PO.001) .In contrast with the healthy group,there was significant difference between the climax stage group and the convalescence stage group (p<0.001),but no diference to the catabasis group (p>0.05).The difference among the climax stage group ^ the nephritic nephrisis and the purpuric nephritis is not statistic significant(p>0.05) .However, the climax stage group is higher than the nephritic nephrisis and the purpuric nephritis.3. All groups of NS exhibite higher levers of serum EAthan the healthy group(p<0.05).The climax stage group of SRSNS was the highest (77.4304±17.63662)ofall( (PO.001) .In contrast with the healthy group,there was significant difference between the climax stage group and the convalescence stage group (p<0.001),but no difference to the catabasis group (p>0.05).The difference among the climax stage group-. the nephritic nephrisis and the purpuric nephritis is not statistic significant(p>0.05) .However, the climax stage group is higher than the nephritic nephrisis and the purpuric nephritis.There was a significant correlationamong the quantification of urinary proteuu Urinary GAGs > Hpa and EA. Multiple linear regression analysis showed that the relevance between the quantification of urinary protein and Hpa are positive linear correlation^ R= 0.953, p<0.05). Among of Urinary GAGs, Hpa and EA, there are positive linear correlation (R=0.851, PO.001) by multiple linear regression analysis.Conclusion:There was a significant correlation among the quantification of urinary protekK urinary GAGsn Hpa of peripheral blood leukocyte and serum EA.The relevance among of urinary GAGsn Hpa and EA are positive linear correlation.In the NS,the urinary protein may be resulted by the spallation of GAGs within GBM.The spallation of GAGs within GBM are possible related with the role of Heparanase of Peripheral Blood Leukocyte and serum Elastase.
