Research On The Location Of Sentinel Lymph Node And Micrometastasis In Cancer Of Oral Cavity

For the patient with cancer of oral cavity,with a high risk rate of cervical lymph node, the histopathlogic status of cervical lymph node is an important prognostic factor. Performing radical neck dissection (RND) is routinely with the presence of N + neck,while the management of the NO neck is controversial, that depends on a reliable pre-operation neck staging. We examined the location and predictive ability of sentinel lymph node biopsy in different layers surrounding the carcinoma by dye-guided method. We studied 25 untreated patients with cancer of oral cavity (cNO). Each of them underwent the RND, with the injection of carbon dye(india ink) pre-operation, and then, after operation, the SLN sent to histopathologic exam with other lymph node. The conclusion is that the sentinel lymph nodes in different layers surrounding the carcinoma mostly locate in the â… ,â…¡ and â…¢ areas. Based on our foundings, the location of the sentinel lymph nodes is not fixed. Meantime, the blacked sentinel lymph node is efficiency to predict the neck status of cNO patient with oral squamous cell carcinoma.Quite a few of the NO phase oral squamous cell carcinoma patients died of recurrence and metastasis because of the micrometastasis. This study usedthe immunohistochemistry stain examine 9 casas postoperative lymph nodes with reaction hyperplasis and found the 7 cases whose lymph nodes with reaction hyperplasis had micrometastasis. From the result, we should use the immunohistochemistry stain to examine the lymph nodes with reaction hyperplasis and evaluate the cervical lymph nodes of NO phase oral squamous cell carcinoma comprehensively. Thus we can evaluate the prognosis and the clinical stage aecurateUy,and direct our clinical work.
