Objective: Nuclear factor-κB(NF-κB) is an important nuclear factor in cell, which plays a vital role in regulation for cytokines-induced genic expression and take part in immunol and inflammatory response. In recent years, nuclear factor-κB has become a hotpoint in medical research about function and mechanism in severe acute pancreatitis. This experiment was to investigate the potential role of NF-κB in inflammation signal transduction and the relationship between its response and lung injury, and pioglitazone, a ligand of Peroxisome proliferators-activated receptor gamma(PPARγ), propably has cural function for severe acute pancreatitis by observing the change in nuclear factor-κB(NF-κB) and the change of NF-κB expression after being-treated with pioglitazone. Methods: Fifty four Sprague-Dawley(SD) rats were randomly divided into three groups: Sham-operated group srved as control(C),severe acute pancreatitis group(A) , and severe acute pancreatitis treated with pioglitazone (I). The pulmonary and pancreatic tissue was observed pathologically from microscopy , serum amylase was determined through the blood of abdominal aorta and the NF-κB expression of lung was determined by immunohistochemical staining. Results: 1,It was obsevered from microscopy that pancreatic and pulmonary tissue of C group had no imflammatory response, there was intensively imflammatory response in A group and imflammatory response was alleviated significantly in I group. 2,Serum amylase of C group decreased after operation, and had no significant difference at times(p>0.05) . Serum amylase of I group decreased after 6 hour. Serum amylase of A group increased continuously after operation. Compared with C group in the same time , serum amylase of A group at 6 and 12 hour and I group at 3 and 6 hour had significant difference(p<0.01). 3,C group had no expression of NF-κBp50. A group had intensively positive response in cytoplasm, and the proportion of positive immunol production had marked hoist(p<0.05) in contrast to C group. In the A group, NF-κBp50 in pulmanary tissue was started increasing at 3 hour, peaking at 6 hours and decreasing at 12 hours. The expression of NF-κBp50 was significantly lower in I group than that in A group(p<0.05). Conclusion: 1,The degree of pancreatic pathology was alleviated and the activity of amylase decreased after being-treated with pioglitazone, which indicated that pioglitazone could brought down acute pancreatitis. 2,The activity of NF-κB in pulmonary tissue increased significantly. The expression of NF-κBp50 was inhibted notedly after being-treated with pioglitazone, which showed that pioglitazone probably played the anti-imflammatory role inhibting the activity of NF-κB.
