Psoriasis (PS) is a chronic inflammatory dermatosis involving the interaction of multiple genes with environmental triggers. There is a clear difference between ethnic groups, pedigrees, which means heterogeneity. Shandong is a province with high prevalence in PS. The aim of this study is to investigate the character of PS in clinical epidemiology and its susceptibility loci.Part One. Clinical Epidemiological Report on FamilialPsoriasis VulgarisObjectives: To investigate the clinical epidemiological character of familial psoriasis vulgaris in Shandong province, Using the method of clinical epidermiology, we can obtain information on the causes and general characters for PS, and try to find out the effects of environmental risks factors on the pathogenesis of psoriasis.Methods: A total of 38 pedigrees of familial psoriasis were collected in Dermatology Department of Qilu Hospital in Shandong University from May 2004 to December 2004. All the patients are Chinese Hans living in Shandong province. To find the effects of environmental risk factors on the onsets of PS, 118 patients of these pedigrees were investigated for the clinical epidemiological characters. The difference between male and female on the age of first onset of PS was analysis by using of Independent-sample t-Test software in SPSS.Results:1. The average age of the first onset was 21.74 + 7.44 for males, and 17.87 +6.66 for females. The average age of first onset of PS was earlier in females than that in males in familial psoriatic patients (t=2.943,P=0.004) .2. Seasonal factors may play important roles in pathogenesis of PS.3. Among all the cases, excluded for those with unknown reasons, the onset and aggravation of PS is associated with infection, trauma, psychic factors, wetness and diet.4. PS occurs in association with other inflammatory diseases such as diabetesmellitus (11%) , hypertension(14%), arthritis(22%). Conclusions:1. The average age of first onset of psoriasis is earlier in females than that formales in familial psoriatic patients.2. Environmental factors may play important roles in pathogenesis of PS.Part Two. Study of Susceptibility Loci of Familial PsoriasisVulgarisObjectives: To elucidate the genetic basis of PS and search for the susceptibility genes of Chinese psoriasis patients, seven microsatellite markers (STRs) on chromosome 4,6,17 were studied, This is to establish whether the seven reported STRs are linkaged to psoriasis vulgaris in Chinese Hans in Shandong province.Methods: We determined to search for the susceptibility genes of patient with psoriasis in Chinese Hans. Seven polymorphic microsatellite DNA markers(D6S276, D6S1610. D4S403, D4S424> D4S415* D17S949, D17S784, ) were selected, Under sample providers' agreement, genomic DNA were extracted from blood of 75 patients and 51 unaffected family members of 22 pedigrees.The samples were first fluorescent-labeled PCR and sequenced with ABI3730 sequencer. The STR genotyping were then carried out using the accompanying GeneScan and GeneMapper software. Linkage study on the genotyping data wasconducted via GENEHUNTER. Results:1. The maximum two-point LOD score on locus D17S949. D17S784. D6S1610 is 2.10s 2.10. 1.04; andtheNPLmaxisl.54. 2.10, 1.35 (PO.05), The results indicate imputative linkages of the genes to these regions. The maximum two-point LOD score on D6S276 is 0.54, and the NPLmax is 0.47 (PO.05), The result indicate putative linkages to these regions.2. The maximum two-point LOD score on locus D4S415. D4S403 is -0.22. -0.06 on chromosome 4q21~4q31,and the NPLmax is 1.32? 0.68 (PO.05) ; The maximum two-point LOD score on locus D4S424 is 0.17 , and the NPLmax is 0.47(PO.05 ),The result indicate putative linkages to these regions.Conclusions:1. It is shown that there be a psoriasis susceptibility gene in 17q23~17q25 and 6p21~6p22 regions respectively in Chinese Hans of Shandong province.2. It is shown that there may be psoriasis susceptibility genes in 4q31~4q34 region in Chinese Hans of Shandong province.
