Objective: To observe the relationship between serum leptin level and the prognosis of sepsis patients, and to investigate the influence of serum leptin to the prognosis in sepsis patients. Methods: 30 male patients who met sepsis criteria according to 2001 SCCM/ESICM/ACCP/ATS/SIS International Sepsis Definition Conference, were studied after they were hospitalized in ICU of Tianjin medical university general hospital from December 2003 to November 2004. Patients with previous history of diabetes mellitus, malignant disease, and of adrenal and pituitary disease, previous history of prolonged use of exogenous corticosteroids, or current therapy with steroids were excluded. There were no difference in BMI in all the patients. 30 male patients were brought into the investigation in the end . We collected clinical variables of the patients after they were hospitalized within 24 hours, based on a singal factor logisticregression analysis to define the risk factors, then we performed the mulvarity logistic regression analysis and found independent risk factors which had important effects on clinical prognosis. There are four independent risk factors: the serum leptin, APACHE II score, cor and glucose. We divided the 30 patients into two groups (nonsurvivor group and survivors group) according to the prognosis and compared the different between the two groups. Results: Serum leptin, APACHE II score, cor, glucose are four independent risk factors in male sepsis patients. There are negative correlation between the serum leptin and APACHE II score (-0.389) glucose(-0.470), cor(-0.528).In Logistic regession the SE value are leptin=0.853,glucose=0.227,cor=0.120, APACHE II =0.687.It shows that serum leptin level and APACHE II have more influence to the prognosis of sepsis patient. The true negative of serum leptin to the prognosis of sepsis is 75%;the true positive is 71.4%. The true negative of APACHE II score to the prognosis of sepsis is 68.8%; the true positive is 78.6%.There are no significant different in judgingprognosis between serum and APACHE II .There are significant difference in serum leptin (survivors 13.89±9.31 ng/ml,...
