Objective: To study the effect of autogenous oxygenator CPB and artificial oxygentor CPB on SIRS Methods: 12 adult mongrels (weight 13 ± 1kg) were randpmly divided into two groups, 6 for experimental group, in which the dog's own lung was used as oxygenator;6 for control group in which the artificial lung (bubble oxygenator) was used as oxygenator. In Experimental group, The CPB system was established by abdominal aorta and left atrium cannulation (for the left~heart bypass), pulmonary artery and right atrium cannulation (for the right~heart bypass). In Control group the CPB system was established by right atrium and abdominal aorta cannulation. The technique of myocardium protection and cardiac arrest were same in both groups. CPB was controlled running for 1 hour in both groups. In experimental group, the lung ventilation were kept during CPB;In the duration of CPB,artificial oxygenator by the adjustment of the Q/V to 1:0.5, to prevent the arterial PaO2 to increase too much. The blood samples were collected in both groups at the time point of pre~operation, 30min of aotic on~clamping, 5min, 60min, 1hour, 2hour, 4hour of post aotic off~clamping. The neutrophil count, platelet count were measured. Plasma concentration of C3, IL-6, IL-8, IL-10,TNF-α was detected with ELISA. Results: After the running of CPB, Platelet count and plasma concentration of C3 were significantly decreased at different time point and were significantly increased in the Experimental groups compared with those in the Control group(sP<0.05);The PMN and plasma levels of IL-6, IL-8, TNF-α were significantly increased at different time point in the Control group compared with those in the experimental groups(P<0.05). However, they were significantly higher than that before CPB in both group .The anti~inflammatory IL-10 concentration was also increased significantly, but there was a lower IL-10 level in Control group than that in the Experimental groups(P<0.05=. Conclusion: The artificial lung's foreign body surface are the main induced factor of the SIRS during CPB. The autogenous oxygenator CPB technique could inhibit inflammatory response in multiple aspects.
