Objective:To investigate the distribution of Helicobacter pylori(Hp)cagA,vacA,iceA,and serum CagA antibodies in patients with gastroduodenal diseases in FujianProvince and explore the relation of Hp strains with chronic gastritis,peptic ulcer andgastric carcinoma. Attempts were also made for the detection of cagA,vacA,iceA andtheir relationship in each other.Methods:A total of 138 Hp strains was isolated from gastric biopsy samples of 47patients with chronic superficial gastritis,22 patients with chronic atrophic gastritis,49patients with peptic ulcer and 20 patients with gastric carcinoma.The cagA,vacA iceAiceA genotypes were determined by polymerase chain reaction(PCR), and detected thesequence to validate them. Serum CagA antibodies in patients with chronic superficialgastritis, chronic atrophic gastritis, peptic ulcer and gastric carcinoma were tested byenzyme immunoassay(EIA) with CagA Hp IgG EIA Kit.Results:81.16% of the strains with cagA gene were detected. vacAs1am2 weredetected in all the Hp strains. The total positive rates of iceA1 and iceA2 were 41.09%and 47.83%,respectively, 36 of isolated Hp strains(26.1%) were positive for both iceAalleles and 43(31.16%) were negative for both iceA alleles. Positive rates of iceA1and iceA2 were higher markedly in the patients with peptic ulcer than those withchronic gastritis and gastric carcinoma(P<0.05).iceA has much relationship withcagA. Total positive rate of serum CagA antibodies in 116 patients was 89.66â„….Positive rates of CagA antibodies in patients with chronic superficial gastritis,chronic atrophic gastritis, peptic ulcer and gastric carcinoma were73.68â„…,86.96â„…,100â„…,100â„…,respectively.The difference of positive rates of CagA antibodies amongthese three groups was not significant (P>0.05).Conclusions:There was no correlation between vacA and cagA genotypes, CagAantibodies and various gastroduodenal diseases. There was significant correlationbetween iceA1,iceA2 and peptic ulcer.Pressence of cagA gene may be necessary foriceA1.
