Objective To investigate the expression of costimulatory molecules CD40 and CD40L in patients with Henochï¹£Schonlein purpura(HSP) and explor their possible significance. Method Research object: 30 HSP patients.the first group is 12 HSPN patients;the second is 18 NO-HSPN patients and the Control is 20 health children. Applying reverse transcription-polymerase chain reaction(RT-PCR) technique,use PHA and Dex as intervention to quantitatively analyze CD40mRNA and CD40LmRNA expression on PBMC(peripheral blood mononuclear cells); to analyze Relation between CD40mRNA ,CD40LmRNA and cytokine IL-6 and IL-8 as well as some clinical information. Results RT-PCR:Compaired with normal controls, the expression of CD40LmRNA increase obviousely in HSP espacially in HSPN but there is no salient increment when CD40mRNA expression is concerned. the CD40LmRNA expression of all in PBMC increase obviousely especially in HSPN after add in PHA and then decrease to normal after add in antiCD40LmAb. the CD40LmRNA expression of the controls in PBMC decrease little but decrease obviousely in HSP espacially in HSPN after add in Dex ,and the CD40mRNA expression in PBMC of the three groups decrease little after add in Dex. The relation between the expression of CD40mRNA,CD40LmRNA and IL-6,IL-8 in serum :the expression of IL-6 and IL-8 in HSP especially in HSPN are more than the controls.then decrease to normal after add in antiCD40LmAb; Relation with clinical information:Expression of costimulatory molecules in all 30 patients with HSP has no significant correlation with the level of IgA in the blood.CD40L expression of 16 patients with HSP whose urine microalbumin within 24 hours(24h UMA)is elevated are significantly higher than those of the other patients whose 24h UMA is normal,and there were positive correlation between expression of CD40L and 24h UMA,a sensitive marker of renal injury,in these 14 patients. Conclusion the abnormal expression of CD40L may play a pathological role in the development of HSP.Having positive correlation with 24hUMA. also the expression of CD40L Have positive correlation with IL-6 and IL-8 in blood serum may be utilized as an immunological marker to evaluate the condition of patients with HSP and detect renal injury at earlier period.
