Objective carcinoma of endometrium is one of three malignant tumor in women reproduction system,in the recent twenty years,the incidence rate of endometrial carcinoma which affect the women health severly have a rising trend throughout the world.But until now we have no precise knowledge of the mechanism how endometrial carcinoma happened.Recenty studies discover anti-oncogenes play an important role in the occurrence,development,incursion and metastasis of endometrial carcinoma.In the present many anti-oncogenes have been evaluated and cloned.Moreover experts have a profoud investigation of the structure and function of these genes.FHIT is one of these anti-oncogenes and its extensive deletions cause our attention in the cancer.The investigation of FHIT could provide a new clue to occurencent mechanism of endometrial carcinoma.Study indicated that the reduced expression of FHIT has close relation with the occurrence,invasion and metastasis of endometrial carcinoma.The purpose of this study was to discuss the expression of FHIT in human endometrial carcinoma tissue and its relation with the invasion and metastasis of endometrial carcinoma and provide themry evidence for the anti-tumor therapy. Method To detect the expression of FHIT immunohistochemistry were used in endometrial carcinoma tissue of 36 patients and control group composed of 14 normal endometrial samples and 17 EIN samples. All specimens come from the first hospital of jilin university (2001-2004)and were confirmed by pathology and which weren't hormone therapy before sugery in three month.According to standard of FIGO,the histology type of all the cases were endometrioid adenocarcinoma and included different pathology degress and surgical pathologic staging.There are 6 cases have lymph node metastasis. Result's standard Observing results make use of double blind procedure.The positive expression of FHIT show brown particles in cytoplasm or nucleus.Refer to Hao'comprehensive scoring method:observe the distrbution,positive strength and positive rate of FHIT protein.First according to the degree of stain score zero, one,two and three respectively.Second according to precent of positive cell score zero(negative),one(75%)respectively.When production of multipliction of precent of positive cell and degree of stain is >or=2,response to immunization is positive.Compare staining reaction of cancer constitution with staining reaction of mucous membrane and epithelial tissue which is normal or no cancer.By optic microscope rough select two sight that positive proportion is bigger.By picture analyzing apparatus estimate percentage andaverage absorbency of positive cell to compare and confirm expression of FHIT protein,which is under object glas 40×and select 3967(μm2) as reference area. Use software SPSS11.0 to analyze results of immunohistochemistry which is divided into groups according to different clinical pathological marker,and make use of X2 test to compare rate among these groups. Results 1.The FHIT protein expression of normal endometrial tissue,EIN and endometrial carcinoma:The positive expression proportions of FHIT protein in normal endometrial tissue is 92.86 %(13/14),specific pigmenting is in cytoplasm or nucleus where is diffuse distribution.The positive expression proportions of FHIT protein in endometrial carcinoma tissue is 50%(18/36) that is point -sheet or regional distribution.The positive expression proportions of FHIT protein in EIN is 64.71%,distribution condition is between two above-mentioned conditions.There is significance among three groups(P<0.01) 2.Reduced expression of FHIT protein is associated with surgical pathologic staging,histological grade,lymph node metastatic and depth of myometrial invasion in EC. Reduced expression caicinomas of FHIT protein in histological grade ⅠⅡⅢis 57.89%(11/19),50% (5/10),28.57%(2/7)respectively,the difference among grade ⅠⅡⅢis none of significance(P>0.05). Expression of FHIT protein in surgical pathologic staging of EC is64%(16/25)in stage Ⅰ,20%(1/5)in stageⅡ,16.67%(1/6)in stageⅢ-Ⅳ,there is significance among three groups(P<0.05) .The positive expression of FHIT protein with lymph node metastatic and no lymph node metastatic is 16.67%(1/6),66.67%(14/21)respectively,there is significance between two groups(P<0.05).The positive expression of FHIT protein with Superficial myometrial invasion and with deep myometrial invasion in EC is 65%(13/20),31.25%(5/16) respectively, there is significance between two groups(P<0.05). Conclusion 1.FHIT protein express in normal endometrial tissue,EIN and endometrial carcinoma.The positive expression proportions of FHIT protein in endometrial carcinoma tissue were lower than normal endometrial and EIN samples.(P<0.01).2.Except the expression of FHIT protein in histological grade descends,but there is no significance among three groups(P>0.05),the expression of FHIT protein in tumor cells were significantly associated with the presence of mayometrial invasion and surgical pathologic staging and lymph node metastasis(P<0.05).This suggested the reduced expression of FHIT are closely related with the invasion and metastasis in endometrial carcinoma.
