Objective: To determine if detection of Livin mRNA expression in tissue and urine by using RT-PCR can provide a novel tool for the diagnosis and therapy of transitional cell carcinoma of the bladder . Methods: Livin mRNA in T24 bladder cancer cell line, tumoral tissues of 51 patients with TCCB, 10 normal bladder tissues, exfoliated urinary cells of 52 patients with TCCB, 20 patients with urinary cancers except bladder, 10 nonneoplastic urinary diseases patients and 10 healthy volunteers were detected. Livin and β-actin were amplified by using RT-PCR. Results: 1. T24 bladder cancer cell line showed positive Livin αmRNA signal. 2. The positive rates of Livin α in TCCB and normal bladder tissues are 70.6% and 0% respectively . The expression level of Livin α mRNA is elevated as the grade and stage increased in TCCB,The positive rates in G1,G2,G3 are 45.0%,84.0%,100.0%,in stage of Tis-T1,T2-T4 are 45.8%,92.6% and in primary,recurrent cases are 56.7%,90.5% respectively, (p<0.05);while none showed positive Livin βsignals in TCCB and normal bladder tissues. 3. Only 2 of 40 (5%) showed positive Livin α mRNA signal in urine from control group. 45 of 52 (86.5%) patients with TCCB showed positive Livin α mRNA signals in urine. The Livin α mRNA expression difference between experimental group and control group was significant (p<0.05). The positive rate of Livin α mRNA in T1 was 88.9% (24/27),T2-4 84.0%(21/25);G1 88.0%(22/25),G2 83.3%(15/18),G3 88.9%(8/9);while none showed positive Livin β signals in urine in TCCB and control group. 4,Detection of Livin mRNA expression in tissue and urine in 34 patients with TCCB at the same time, The positive rates of Livin α mRNA in tumor tissue and urine are 70.6% and 85.3% respectively(p>0.05). Conclusions: 1. The up-regulated expression of Livin α mRNA in TCCB suggests that Livin α mRNA is tight related with the tumor occurrence and progress.Besides,the higher the grade and stage,the higher the expression level of Livin αmRNA.Furthermore, Livin α mRNA is correlated with recurrence of TCCB.therefore,we concluded that Livin α mRNA might be the indexs for identifying grade,stage and recurrence of TCCB. Livin α mRNA can provide new appropriate target for gene therapy to TCCB. 2.Highly sensitive and specific determination of urine LivinαmRNA appears to provide a simple, noninvasive diagnostic test to identify patients with TCCB. There is no relationship between LivinαmRNA and the stages or grades of the TCCB. 3.There is no difference between the Livin α mRNA expression of tumor tissue and urine.
