| Objective:To investigate the diagnostic value and spiral CT enhance characteristics of lymph node metastasis of gastic carcinoma with triphasic incremental spiral CT of the stomach.Methods:Both plain scan and triphasic incremental spiral CT of upper abdomen were performed in 73 patients with progressive gastric carcinoma,the results of SCT were compared with the surgical and pathologic findings in all cases;The diagonostic results of lymph node metastasis or not of gastric carcinoma were compared between plain san helical CT only and plain san examination with triphasic incremental helical CT.The features of metastaic lymph nodes of gastric carcinoma with spiral CT were analyzied.Results:1.The results of triphasic incremental spiral CT for lymph node metastasis yielded an accuracy of 86.3%,sensitivity of 86.2%,specificity of 86.7%,positive predictive value of 96.2%,and negative predictive value of 61.9%,respectively.2.The diagnosis and detection of metastatic lymph node by triphasic in- cremental spiral CT is better than by plain scan spiral CT only,the difference was statistically significant between the two groups(p<0.05). 3.The main features of metastatic lymph nodes of gastric carcinoma with plain scan and triphasic incremental spiral CT:⑴Substantial entity was displayed mainly in small lymph node, and annular or cystic shape appeared with the size of lymph node getting bigger. ⑵Style of lymph nod enhancement was divided into symmetrical entity, ammulus and cyst. ⑶The degree of lymph nod enhancement which is lower than the blood vessel in venous and atterial phase was difference with the different phases, and the increased extention is 13-36HU.⑷The symptom of lymph node with plain scan spiral CT is accorde with literatures published.
