Objective: To investigate the expression of cyclooxygenase-2 (COX-2) in intrahepatic cholangiocarcinoma and the relationship between the expression of COX-2 and the neoplastic transformation of biliary epithelial cells and the malignant progression of intrahepatic cholangiocarcinoma. Methods:COX-2 protein and Bcl-2 protein were analysed in 30 cases of intrahepatic changiocarcinoma ,hepatolithiasis by immunohistochemical PV9000 method. 30 cases normal liver tissue were analysed as control. CD34 was used to mark micro vascular endothelium cell to count micro vascular density (MVD) in the tissue of intracholangiocarcinoma. ResultsfThe positive rates of COX-2 expression in intrahepatic cholangiocarcinoma, normal intrahepatic bile duct were 83%(25/30),16.7%(5/30) respectively .Their difference was significant(P<0.01). The positive rates of Bcl-2 expression in intrahepatic cholangiocarcinoma, normal intrahepatic bile duct were 73%(22/30),20%(6/30) respectively .Their difference was significant(P<0.01).The mean value of MVD in COX-2 positive group of intrahepatic cholangiocarcinoma was 17.26 ±4.41.The valueof COX-2 negative group was 14.08+2.90. Their difference was no significant.(P>0.05).The expression of Bcl-2 was correlated with the expression of COX-2.(r_s=0.387,P<0 .05,Spearman test).Conclusion: The overexpression of COX-2 might play a crucial role in the carcinogenesis and development in intrahepatic cholangiocarcinoma.Suppressing the apoptosis of endothelium cell in intrahepatic bile duct by activing Bcl-2 might be one of the mechanism of carcinogenesis.The results indicate that COX-2 might provide a target for chemopreventive strategies for intrahepatic cholangiocarcinoma.
