Objective To investigate the changes of plasma thrombomodulin, von Willebrand factor and plasminogen activator inhibitor type 1 in essential hypertensive patients and to discuss the relationship between the molecular markers of endothelial dysfunction and the complications of thromboembolism in essential hypertension.Methods 98 patients with essential hypertension were divided into three groups according to different grades, while 36 healthy persons were selected as healthy controls. The plasma concentration of thrombomodulin, von Willebrand factor and plasminogen activator inhibitor type 1 were all measured by enzyme linked immunosorbent assay (ELISA) method.Results (1) It was found that the levels of plasms von Willebrand factor (VWF) and plasminogen activator inhibitor type 1 (PAI-1) were significantly higher in essential hypertensive patients with grade 1,2,3 group than those in controls (P< 0.05). (2) The levels of plasma thrombomodulin (TM) were significantly higher in essential hypertensive patients with grade 2,3 group than those in controls (P< 0.05), but there was no significant difference between essential hypertensive patients with grade 1 group and controls (P>0.05). (3) The levels of plasma TM, VWF, PAI-1 were significantly higher in hypertensive patients with grade 2,3 group than those in grade 1 group (P<0.05), while there was no significant difference between grade 2 group and grade 3 group (P>0.05). (4) Plasma TM was correlated with plasma VWF and PAI-1 in the patients with essential hypertension (P<0.05).Conclusions (1) We conclude that plasma TM, VWF and PAI-1 are higher in essential hypertensive patients, indicating that those patients had endothelium dysfunction, anticoagulant reduction, platelet activation and impaired fibrinolysis, which may be the pathogenesis of complications such as ischemic stroke, heart attacks.
