Study On Jin Niu Fu Tong Tablet's Functions Against Protective Factors Of Peptic Ulcer

Peptic ulcer (PU)is a frequently-seen disease in clinic .China is among the areas of high incidence. Several clinical studies show that PU is quite damaging to the workforce. At present, the pathological mechanism how PU happens is not complete known yet. It is generally considered that it is the result of the imbalance between the protective factors and damaging factors of the mucosa. In recent years, the researches have suggested that strengthening protective factors of mucosa and promoting the repair of mucosal lesion are important approaches in the management of PU. Thus, the importance of protective factors in mucosal lesion in PU has been increasingly recognized.In the theory of traditional Chinese Medicine (TCM) , there are many reasons why the PU happens, the basic pathological mechanism being the stagnation of qi activity or malnutrition of meridians which leads to the failure of stomach to harmonize and downflow and cause stomach pain. The principle of treatment is regulating qi, harmonizing stomach and stopping the pain. Results in the treatment of PU have been good according to syndrome differentiation. However, studies on the therapeutic mechanisms of such treatment have been rare, especially those regarding protect factors of mucosa.Jin niu fu tong tablet(JN) is developed by Guangdong Provincal hospital of T. CM., which consists of Zanthoxylum nitidum, scutellaria root, licorice root, white peony root, aucklandia root, corydalis tuber, totunda ilex . The tablet has the functions of regulating qi .clearing heat, removing the blood stasis, dredging the channel, smoothing the liver and relieving depression. A research project finished in 1997 showed that the function of JN tablet includes suppressing gastric acid secretion, suppressing the activation ofpepsin, suppressing helicobacter pylori(HP) and stopping pain, and it was effective in the treatment of PU. On the basis of this former research, the current research is to further explore the therapeutic mechanisms through studying the tablet's effects on protective factors of peptic ulcer.Methods and objectives1 .Experimental study. Gastric ulcer models were established through the ligation of pylorus in mice. These model mice were randomly divided into 4 groups : the control group of noral saline, JN tablet group of low dosage, JN tablet group of high dosage, and sucralfate positive control group. Each group contained 10 animals. Model mice were put to death , and their gastric juice were collected and prostaglandin E2(PGE2), epidermal growth factor (EGF) levels were determined through radioimmunoassay(RIA) to study the protective functions of JN tablet against peptic ulcer, elucidating the its therapeutic mechanisms against PU.2 . Clinical research. 80 PU patients were allocated half-randomly into two groups: JN tablet treatment group and sucralfate control group, (50 in the treatment group , 30 in the control group), observation was made on finding a best suited TCM syndrome for the tablet, guiding the selection of patent Chinese medicine in the treatment of PU in clinical practice.Results1 .The results of experiment study show: (1) PGE2levels of the gastric juice of JN tablet group (bothe high and low dosages) are higher than those of the control group, but no difference with those of sucralfate control group.(P>0.05) Meanwhile, PGE2 levels of the gastric juice of high dosage group is higher than the low dosage group. (2) EGF levels of the gastric juice of JN tablet dosage group and sucralfate control group are higher than normal saline control group significantly,but there is no difference between high dosage group and sucralfate control group (P>0.05) .EGF levels in low dosage group is lower than those of the sucralfate control group. In addition, though EGF levels of gastric juice of the high dosage group are greater than those of the low dosage group, there is no statistical significance between these two groups (P>0.05) .2 .The clinical study results shows that JN tablet has remarkable effectiveness in improving the symptoms acid reflex, belching ,etc..
