INTRODUCTIONAcute coronary syndrome ( ACS ) including unstable angina pectoris (UAP) , acute myocardial infarction (AMI) and cardiogenic sudden death. The incidence of ACS obviously raised in recent years. It mostly due to the fracture of plaque, thrombosis up to obstruction of antrum. The result of investigation indicate that inflammation and immunization play an important role in the arises, e-volve and fracture of the unstable plaque. Matrix melloproteinase-9 ( MMP-9) , also named gelatinase B, which is an important member in the family of Matrix melloproteinase ( MMPs). The research has confirmed that MMP-9 could increase the instability of artery atherosclerosis (AS) plaque, accelerate the plaque rupture and thrombosis. The increase of MMP-9 maybe the one of the reason of ACS . Tumor necrosis factor - alpha (TNF-α) is an important cell factor of pre-in-flammation and immunization adjustion,it has a tightly connection with the cardiovascular disease. Some studies show that it can improve the instability of atheromatous plaque. But there has few study about whether it has correlation bewteen MMP-9 and TNF-α.In the present study, we determined the serum MMP-9 and TNFa levels in 49 patients who were proved ACS, analyzed the correlation between MMP-9 and TNF-α, as well as discussed the relationship between inflammation and target of plaque stability.SUBJECTS AND METHORDSSubjects The study groups included 49 cases of ACS patients which divided into two subgroups:26 cases of AMI,23 cases of UAP, and 25 cases normalcontrols. All groups matched mutually for age, sex, smoking, BMI, et al.Measurements We obtained 3-milliliter blood samples in AMI patients by collection of blood from a cubital vein right after in hospital and obtained 3-milliliter fasting blood samples in the morning in UAP and controls. We determined MMP-9 and TNF-α with the quantitative enzyme immunoassay technique (ELISA). Blood grease were detennined with routine clinical laboratory test.Statistical analysis All data were expressed as the mean ± SD. One-wayANOVA was used to compare differences for inter-group analysis. X analysiswas performed to assess the clinical data inter-groups. Pearson correlation wasused to lest the association of MMP-9 and TNF-α. All statistical tests were two- tailed and a value of P <0.05 was regarded as statistically significant.RESULTS1. The serum level of MMP-9 was elevated significantly in group ACS as compared to controls, and serum level of MMP-9 in AMI group was higher than that in UAP group ,the difference was significant.2. The serum level of TNF-α was elevated significantly in ACS as compared to the controls. Serum level of TNF-α in AMI group was higher than that in UAP group, but the difference was no significant.3. The level of serum MMP-9 was closely correlated with the level of serum TNF-α.DISCUSSIONA lot of studies show that ACS is resulted in the atherosclerosis plaque rupture. It is not the size but the character that detennined the stability of plaque. The character of unstable plaque is that it has a larger lipid core,thinner fiber cap, fewer cells of smooth muscle, and more monocytes soakage. The MMPs which excreted by monocytes can decompose extra cellarmatrix, weaken the structure of fibre cap,and lead to the occur of ACS.
