Object:Intrcranial tumor is a common disease in neurosurgery, of which meningioma,glioma,metastatic brain tumor account for over 60%.Although the recent imaging technological advance and microsurgery have improved thediagnosis and treatment of the foregoing brain tumors,some problems still remain. The determination and application of such prognostic indexes as cerebroma soakage recurrence and cancerometastasis is the major problem in clinical practice.Recently molecular biological study on intracranial primary tumor and metastatic tumor shows that prognsis of brain tumor is closely associated with vascualr endothelial growth factor(VEGF) and urokinase-type plasminogen arctivextor(uPA) in addition to clinical stage, pathological grading and peritumoral brain edema. Little research has been done in this field although there are some relevant reports.this study chooses intracranial primary and subsequent, benign and malignant meningioma, glioma,metastatic brain tumor whose ways of growth vary but are representative.The relation between their VEGF and uPA expression and their clinical pathology and peritumoral edema are detected and checked in order to provide guidance for clinical treatment or prognosis.Method: 100 patients with meningioma (30 cases) ? metastatic brain tumor(30 cases) and 40 patients with glioma were hospitalized in the department of neurosurgery from Oct, 1998 to Oct,2004.All the cases were diagnosed accurately by two plus pathologists.According to WHO Glioma Grading ,8 cases were I grade ,12 cases II grade, 10 cases â…¢ grade and 10 cases â…£ grade .100 patients were followed by follow-up study(in 6 years).Immunohisological chemical dye was applied to all the tissues to be checked using streptavidin-Peroxidase method(SP method). The deggree of peritumoral brain edema (PTBE)was estimated on the basis of CT of head and MRI .The results of all the groups were analyzed statistically.Results:Among 100 patients with brain tumor, the total positive rate of VEGF when immunohistological chemical dye was applied was 90%(90/100 cases) while the positive rate of VEGF was 76.67% for meningioma, 95%for glioma and 96.67% for metastatic brain tumor respectively. The total positive rate of uPA when immunohistological chemical dye was applied was 91%(91/100 cases) while the positive rate of uPA was 83.33% for meningioma, 92.5% for glioma and 96.67% for metastatic brain tumor respectively. The positive rate of VEGF and uPA for glioma and metastatic brain tumor is significantly higher than that of VEGF and uPA for meningioma, and the show is consistent in each tumor .VEGF and uPA could not show in 10-case without tumor involvements and normal brain tissues.The show of VEGF and uPA is posistively correlated with the malignance level of glioma . The show of VEGF and uPA is lower in low-graded glioma and higher in high-graded glioma. The VEGF show (5%/35%)and uPA {5%/40%}show of low-graded glioma and high-graded glioma were showed positively and showed significant difference.Brain edema was most obvious in patients with metastatic brain tumor,less obvious in patients with glioma,and least obvious in patients with meningioma.
