PrefacetThe incidence of breast tumors in China is rising along with the increase of material living level. So it' s becoming more and more important to differentiate benign and malignant breast tumors in time and accurately. One of the widely used breast examination method is the Molybdenum target X - ray imaging, but this method has many limitations. MRI was used in breast examination as early as in the 1980' s, but because of the high price for special breast coil, it' s only applied in few domestic hospitals.A newly established SENSE - body coil, which is PA surface coil for body exams, has the advantage of getting high homogeneity in larger scan range, thus can be used to acquire breast images at same resolution as conventional single -side breast substitute coil. But this advantage of SENSE - body coil in breast imaging is not widely noticed, and no literature in its application experience is found.Because of the current situation of clinical application of breast MRI imaging, this study uses SENSE -body coil in the clinical examination of breast, comparing its ability in the differentiation diagnosis of benign and malignant breast masses to routine Molybdenum target X - ray imaging, so as to push the further clinical application of breast MRI imaging.Materials and Methods1. General materials. Collecting breast mass patients found by palpation from June 2004 to December, 2004, 30 patients with 33 masses. All patients arefemale, with average age of 41. All patients undergone Molybdenum target X ray imaging and MRI plain and enhancement scan, and gaining pathology.2. Examination equipments. X - ray images are taken by Molybdenum target breast imager from Hologic company, data read by Philips CR system. MRI images are taken from Philips New Intera 1. 5T superconducting MRI scanner, with SENSE - body coil and self - made breast support apparatus.3. MRI scanning method. Prone head first, breast gland overhang spontaneously into the hole of SENSE - body coil and supporting system. Routinely use axial and sagittal section, setting sequences and parameters according to literature.4. Evaluation of images. All X - ray and MRI images are read by 2 radiologists blindly.(1) Diagnosis standard of benign and malignant breast masses in X - ray images: according to the study by Hu Yongsheng, there are 8 direct and 7 indirect signs in diagnosing breast cancer.(2) The malignant signs in MRI plain scans are: unclear margin of tumor, with burrs, irregular shape, other signs are judged as benign.(3) Malignant signs in enhanced MRI scan: peak enhancement rate (SIR) ≥90% , or 60 ≤ SIR < 90 plus unclear margin or irregular shape, others are considered as benign.(4) Malignant standards in dynamic enhanced MRI scan: dynamic shape is of type A, or type B plus unclear margin or irregular shape, others are considered as benign.Results1. 30 cases with 33 masses. Pathology diagnose 8 cases with 9 masses are malignant, 22 cases with 24 masses are benign.2. The diagnosis sensitivity, specificity, accuracy, positive predictive value and negative predictive value of X - ray are 66. 7% , 87. 5% , 81. 8% , 66. 7% , 87.5% respectively, X~2 =4.41 >3.83, P <0.05,statically significant.3.
