Objective: Which provides a new option to study cerebralvascular disease. Leptin is a product of obesity, which plays an important role in the regulation of food intake and metabolism. Leptin in a mediated-hormone of the modulation mechanism in fat-insulin axis. Insulin resistance (IR) is a phenomenon that biological reaction to insulin in the body in lower than expected, associated with HIS. IR has been found to be correlated to a number of metabolism failure. Other researches show that IR is dangerous factors of cerebrovascular disease. The relationship between leptin and IR become a hotspot in recent years, but few reports can be seen in domestic. Fasting serum leptin, fasting insulin and fasting blood glucose were measured, To study the relationship between serum leptin level, insulin resistance and irritable hyperglycemia within stroke, to investigate the role of serum leptin and insulin resistance in stroke and irritable hyperglycemia after stroke. Method: Fasting serum leptin (LEP), fasting insulin (FINS), fasting blood glucose (FBG), insulin sensitivity index (ISI) and body mass index (BMI) were determined in 102 cases of patients with stroke in one week (60 cases in high FBG group, 42 cases in normal FBG group, having no diabetes history in the past) and 21 cases health persons of control group, LEP and FINS concentration by radioimmunoassay. According to the FBG values of stroke patients, it is divided into two groups: high FBG group (FBG>6.1mmol/L) and normal group (FBG≤6.1mmol/L). Recorded lesion quality, lesion position, lesion size of patients with high FBG group according to CT or MRI, it is divided into CI group and CH group; brain stem group, thalamus group, basal ganglia group, brain lobe group; large size group, middle size group and small size group. Analyzing the relevance during these values above. Results: The values of each group were expressed by x±s. FINS, FBG in the stroke patients (16.08±4.69mU/L, 7.19±2.17mmol/L) were significantly higher than those in the control group(9.39±1.84 mU/L, 5.11±0.49mmol/L); LEP in the stroke patients (8.92±3.66ng/ml) (men 7.52±2.25ng/ml, female 10.17±2.17ng/ml)were significantly higher than those in the control group (4.92±1.53ng/ml) (men3.95±1.73ng/ml, femen6.48±3.13); ISI in the stroke patients(-4.65±0.58) were significantly lower in the control group(3.85±0.24) (P<0.001); LEP in the stroke men patients and the men control group were significantly lower in the female patients and the female control group (P<0.05). The LEP level was related positively to FINS in the stroke patients (men r=0.389, P<0.01; female r=0.489, P< 0.001); The LEP level was related negatively to ISI in the stroke patients (female r=-0.504, P<0.001; men r=-0.465, P<0.001). FINS and FBG in the high FBG group(17.7 ±7.83mU/L, 8.60 ±1.72mmol/L) were significantly higher than those in the normal FBG group (13.67±4.80 mU/L, 5.18±0.54mmol/L); LEP in the high FBG group (10.05±3.54ng/ml) (men 8.33±1.60ng/ml, female11.13±4.49ng/ml) were significantly higher than those in the normal FBG group (7.35±3.41ng/ml) (men 5.58±1.83ng/ml, female 8.65±2.77 ng/ml); ISI (-4.99 ±0.34) in the high FBG group were significantly lower in the normal FBG group (-4.17±0.51) (P<0.001). The FBG level was related positively to INS in the high FBG group (r=0.423,P<0.001) ; the FBG level was related negatively to ISI in the high FBG group (r=0.878,P<0.001); but the FBG level was not related with LEP in the high FBG group (men r=0.069,P>0.05; female r=0.299, P>0.05).
