Objective To study the effect of carvedilol, a new type β adrenergic receptor blocker, on glycometabolism and lipid metabolism as treated in gerontism patients with chronic heart failure . Methods Since March 2002 to June 2003, 60 patients who had chronic heart failure with diabetes mellitus, Among them, 53 were male, 7 were female, and their age was over 65 years old. They were randomly assigned 30 patients to treated group and 30 patients to controlled group. According to New York Heart Association cardiac functional classification, patients were diagnosed heart failure and classified. Among them, class II were 30 patients; class III were 24 patients; stable class IV were 6 patients. Controlled group were treated with conventional therapy: diuretics, angiotensin converting enzyme inhibitors and digitalis if it was necessary. Treated group were treated with carvedilol and conventional therapy. Patients received an initial dose of 3.125mg of carvedilol twice daily for one week, which was then increased at two-week intervals (if tolerated), first to 6.25mg, then to 12.5mg and finally to a target dose of 25mg twice daily. This dose was maintained for 8 weeks. Results There were not significant difference before they were selected between two groups in age. blood pressure, heart rate, blood sugar and cholesterol and triglyceride. There were significant difference before and after treatmentin two groups in systolic blood pressure(P<0.005), diastolic blood pressure(P<0.001), heart rate(P<0.05), fasting blood sugar and postprandial blood sugar (P<0.05),cholesterol and triglyceride (P<0.05). Conclusion Carvedilol was efficacy and safety in gerontism patients. It was not adverse on glycometabolism and lipid metabolism in gerontism patient with chronic heart failure with diabetes mellitus.
