Objective: To study the treatment effects and mechanism of carthamus tinctorius L injection on severe acute pancreatitis (SAP) in the rat. And to examine whether these effects are dependent on intravenous or local arterial administration in different time.Methods: 70 healthy male SD rats were randomly divided into 7 groups(10 rats for each group): GroupⅠ, sham operation,animals treated with intravenous saline solution; GroupⅡ, SAP model, animals of SAP treated with intravenous saline solution; GroupⅢ, animals of SAP treated with intravenous nafamostat (1.5mg/kg·h-1, treatment started 3 hours after the induction of SAP); GroupⅣ animals of SAP treated with intravenous carthamus tinctorius L injection (0.5ml/kg·h-1, treatment started 3 hours after the induction of SAP); GroupⅤ, animals of SAP treated with intravenous carthamus tinctorius L injection (0.5ml/kg·h-1, treatment started 0.5 hours after the induction of SAP); groupⅥ animals of SAP treated with intra-arterial carthamus tinctorius L injection (0.5ml/kg·h-1, treatment started 3 hours after the induction of SAP); GroupⅦ, animals of SAP treated with intra-arterial carthamus tinctorius L injection (0.5ml/kg·h-1, treatment started 0.5 hours after the induction of SAP). The animals were sacrificed at 12 hours following SAP, Every group animals received a saline solution infusion of 8 ml/kg·h-1.The blood samples were collected for measurement of serum amylase,TNF-α,plasma TXB2, The pancreas and lungs were partly taken for measurement of MPO,pathologic observation,pathologic assessment with Hughes scoring system,Ascites of each group was accumulated.Results: The experimental result were as follow: Compared with groupⅠ,each survey was higher in other groups(p<0.001); The level of serum amylase,serum TNF-α,plasma TXB2,the content of MPO of pancreas and lung,pathologic scoring of pancreas and lung in all treatment groups were obviously lower than those of groupⅡ (p<0.05). The level of serum amylase in groupⅢ was lower than that in group IV (p<0.01), but The level of plasma TXB2 in group IV was lower than that in groupⅢ(p<0.02), other survey in those groups had no difference. The level of serum amylase,serum TNF-α,plasma TXB2,the content of MPO of pancreas,pathologic scoring of pancreas and ascites volume in the intra-arterial group(ⅥandⅦ) was lower than those in the intravenous group(ⅣandⅤ) (p<0.05), respectively. but the content of MPO and the pathologic change in lung in groupⅣandⅤ was obviously decreased(p<0.05). The level of serum TNF-α,plasma TXB2,the content of MPO of pancreas and lung, pathologic scoring of pancreas and ascites volume in the early groups(Ⅴand Ⅶ) was lower than those in the late group(ⅣandⅥ) (p<0.05), respectively.Conclusions: Carthamus tinctorius L injection has obviously treatment effects on severe acute pancreatitis (SAP) of rats, Its effects depend on the route and time of administration; the treatment effects of the intra-arterial route was better than that of the intravenous route; the treatment effects of the early was more effective than that of the late. The mechanism of carthamus tinctorius L injection for treatment of SAP on rats is possibly associated with inhabiting the release of TNF-α,TXB2 and the activity of neutronphil in the pancreass and lung.
