Objective: Evaluation of facial balance is a constant continuous study and learning process for orthodontists. Tooth movement and proper positioning of the teeth to ensure favorable facial changes and to avoid unfavorable changes should be in the orthodontist's "diagnostic" mind from the very first examination. The continuous controversy concerning extraction of premolar teeth and its supposed effects on the facial profile led to the development of this study. Method: Twenty-five patients were treated with four premolars extraction. Every patient took pretreatment and posttreatment cephalographs and tracings. The soft tissue profile were evaluated. Results: There was a significant increased in facial angle, nasolabial angle, Z angle, upper lip length, upper lip thickness and lower lip thickness. The upper lip protrusion was significant decreased.Conclusions: In patients of Angle II division 1 malocclusion with four first premolar extractions, the changes of soft tissue are same as the dental and skeletal pattern -from Angle II to Angle I. The protrusion of soft profile was decreased. The patients with extract treatment had the facial esthelics and facial balance.
