Objective: To investigate the effects of low level He-Ne laser & chinese medicine xiaolingxian ointment on the ischemia-reperfusion in rabbits'knee and offer experimental proofs to prevent or to treat the dysfunction of the joints after replantation in digital or limb amputation clinically.Methods:100 New Zealand health white rabbits,either male or female,weighting 2.88±0.38kg,were randomized into four groups, ischemia reperfusion group(A), low level laser treating group(B), xiaolingxian ointment group(C), low level laser combined Chinese medicine xiaolingxian ointment group(D). 25 rabbits were devided into five sub-groups randomly in each group. There were five time point. Rabbits were sacrificed at 1,2,4,8and 12 weeks after ischemia-reperfusion. Synovia and synovial membrane were gathered to be measured by lactic acid, SOD,MDA,NOS and observed by light microscope,electron microscope and immunochemical method. Results: Under the light microscope, during the earlier period of the ischemia reperfusion in group A, there were many hemorrhagic spots in the synovium with infiltration of inflammation cells. The surface of synovium was destructed accompanied with degeneration and necrosis of the synoviocytes. Moreover,swelling of synovicytes and endothelial cells was obvious. In group B,C and D, the injury were gently, especially in group D. Between 8 weeks and 12 weeks, the surface of synovium was repaired with the hyperplasia of synovicytes and excessive fibrosis in group A. In other three groups, the synovium were normal with more capillary vessel than that in group A.The changes of MDA and lactic acid were parallel in four groups. It gradually decreased from 1 to 12 weeks after IR injury. However, at the same time points, Compared with group A, MDA was significantly lower in group D (p<0,05). On the contrary, the activity of SOD and NOS were increased gradually from 1 to 12 weeks after IR injury in four groups. SOD and NOS in group A were significantly lower in group A than that in other three groups(p<0,05). Blood capillaries were counted by immunochemical method. From 1 to 12 weeks ,in group A, it became lower. But in other three groups, there were no obvious changes at 4,8,12 weeks, the amount of capillary vessel in group A was significantly lower than that in other three groups(p<0.05).At 4,8,12 weeks, the amount of activity in knee in group A was significantly lower than that in other three groups(p<0.05).Conclusions:1. Low level laser and Chinese medicine xiaolingxian can alleviate the ischemia reperfusion injury of the joints by relieve lipid peroxidation, reinforcing antioxidant enzyme.2. Low level laser and Chinese medicine xiaolingxian can facilitate blood capillary regeneration, maintain microcirculation. Moreover, treated with Chinese medicine combined with low level laser,it shows an synergistic effect.
