[Objective] To investigate the difference and their significanceof T cell receptor(TCRVγⅠ—Jγ)gene rearrangement in common hematologic diseases.[Methods] The clonal rearrangements of TCRγ genes were detected by polymerase chain reaction(PCR) and single strand conformation polymorphism(SSCP) in bone marrow cell of 115 patients with diseases of hematologic diseases,including 27 cases of acute lymphoblastic leukemia(ALL),14 cases of malignant lymphoma(ML),37 cases of acute myeloid leukemia (AML),15 cases of myelodysplastic syndrome(MDS) , 9 cases of aplastic anemia(AA), 9 cases of idiopathic thrombocytopenic purpura (ITP),4 cases of immunorelated pancytopenia(IRP) and 3 patients of systemic lupus erythematosus (SLE).SSCP was used to detect the cases that were positive samples of TCRVγⅠ—Jγ gene rearrangement in PCR. The monoclonal or oligoclonal samples were taken to DNA sequencing. Semiquantitative test of TCRVγⅠ—Jγ gene rearrangement was detected in ALL by Gel ANalysis SYStem.[Result] ⑴In patients with ALL,ML,AML,MDS, AA,ITP ,IRP and SLE, the TCRγ gene rearrangement rates were 77.78%,72.22%,37.83%,46.67%,77.78%,66.67%,50.00% and 100% respectively.⑵ The integration of ALL with partial remission and complete remission was significant lower than that with initial treatment(P<0.05). ⑶The patients of monoclonal or oligoclonal with dominant clone were found by SSCP in the malignant diseases and the cases of polyclone or oligoclonal without dominant clone were found in the benign diseases (P<0.05) . ⑷DNA abnormalities were found in some cases with TCRVγⅠ—Jγ gene monoclonal rearrangement. [Conclusions] (1)Detection of the TCRVγⅠ—Jγ gene rearrangement is useful to diagnose lymphocytic malignant disorders and to monitor minimal residual diseases(MRD) .(2)The TCRVγⅠ—Jγ gene rearrangement also can be found in the acute nonlymphocytic leukemia(ANLL) and myelodysplastic syndrome(MDS).(3) The TCRVγⅠ—Jγ gene rearrangement is of potential value to differentiate the malignant diseases and the benign diseases by PCR-SSCP.(4) Different hemapoietic malignant disorders exist in diffenece of TCRVγⅠ—Jγgene sequence.
