Objective: To test whether off-pump coronary artery bypass grafting (OPCAB ) has advantages over conventional coronary artery bypass grafting (CCABG). Methods: Between January 2002 and September 2003, 76 patients with coronary disease underwent only coronary artery bypass grafting by one surgeon. They were divided into two groups, OPCAB group: 46 cases underwent CABG with off-pump and beating heart via sternotomy; CCABG group: 30 cases underwent conventional CABG with extra corporeal circulation. Preoperative and postoperative evariables of two groups were compared. Results: There were no significant difference in sex, age, weight, cute or remote myocardium infarction, hypertension, diabetes and type of bypass grafts of two groups. The number of bypass grafts of OPCAB group and CCABG group was 2.8±0.9 and 2.7±0.9 respectively(p>0.05). Dopamine was infused in 32.6% of the patients in the OPCAB group compared to 70% in the CCABG group (p<0.05). The assisted respiratory time, the volumes of chest tube drainage, the transfusion rates, the intensive care unit (ICU) stay, the hospitalization stay, the hospitalization cost and the operative morbidity of OPCAB group were significant less than those of CCABG group (p<0.05). There were no operative death in either group. All of the patients in both groups were free from anginal symptoms after surgery.Conclusion: Patients with coronary disease underwent OPCAB have less operative morbidity, assiste respiratory time, ICU stay, hospitalization stay and hospitalization cost than those of CCABG. But so far OPCAB can not replace the CCABG, and more clinical trials are necessary for evaluation the middle and late results of OPCAB.
