Research On Concentration Of IL-18, ICAM-1 And Their Relationship In HIE Of Newborn

Objectives: Recently, with the development of molecular biology and molecular immunity, animal experiments and clinical researches have demonstrated that a lot of cytokine, chemokine and apoptosis promotion gene, apoptosis suppressor gene are involved in the course of HIE. IL-18, as a newly discovered inflammatory cytokine, whose role in HIE isn't reported. Particularly, the correlation between IL-18 and ICAM-1 isn't founded .The aim of our study is to detect the action of IL-18 and ICAM-1 in HIE and probe into the co-relationship of them and the state of disease in order to provide a theory for treatment, such as IL-18 binding protein and anti-ICAM-1 antibody.Methods: According to the diagnosis standard and clinical degree of HIE in the fourth nationwide neonate meeting in October 1996 as foundation, 45 cases of term newborn were divided into two groups, HIE group (n=30) and non-HIE group (control group, n=15) who came from the pediatric department of Hebei People's Hospital. HIE group was divided into mild HIE group (n=15) and moderate-severe HIE group (n=15). Infection diseases were eliminated in these cases. Every neonate was drawn 3ml blood from vein on the third and seventh day after hypoxic ischemic. The serum was adopted after centrifugation. At the same condition, the level of all samples of IL-18 and ICAM-1 were examined with Enzyme Linked Immuno Sorbent Assay. Results: (1) IL-18 ①The serum level of IL-18 in control group was 294.19±30.48 (pg/ml). The level of IL-18 in mild HIE group was 695.48±51.61 (pg/ml) whereas that of moderate-severe HIE group was 1287.87±172.76(pg/ml)on the third day.There was significant difference among them (P<0.01). On the seventh day, the serum density of IL-18 in mild HIE group was 351.72±33.36(pg/ml). The density of IL-18 in moderate-severe HIE group was 973.20±113.82(pg/ml). The density was significantly high compared with control group (116.10±27.50pg/ml, P<0.01). ②There was significant difference of IL-18 concentration between mild HIE group and moderate-severe HIE group during the acute and recovery period (P<0.01).③The titre of IL-18 in moderate-severe HIE group in acute period was 1287.87±172.76 pg/ml, which dropped to 973.20±113.82(pg/ml)in recovery period. (P<0.01) .The serum value of IL-18 of mild HIE group in acute period was 695.48±51.61 pg/ml, which declined to 351.72±33.36 pg/ml . There was significant difference between them (P<0.01). (2) ICAM-1 ① The serum level of ICAM-1 in control group was 35.18±3.65 pg/ml, which was significantly lower than those in mild HIE group (55.48±9.69 pg/ml, P<0.01) and moderate- severe HIE group (84.97±13.21 pg/ml, P<0.01) on the third day. On the seventh day, the density of ICAM-1in control group was 19.73±2.57 pg/ml whereas that of mild HIE group was 30.94±9.32 pg/ml. The level of ICAM-1 in moderate-severe HIE group was 50.48±5.98 pg/ml. Significant increase of ICAM-1 level was observed between HIE group and control group (P<0.01). ② During the acute and recovery period , the level of ICAM-1 in moderate-severe HIE group was higher than mild HIE group. There was significant difference between them (P<0.01) ③ As for the same group, the level of ICAM-1 in recovery period descended significantly compared with the acute period (P<0.01), but there was significant difference compared with control group (P<0.01). (3) The data of IL-18 and ICAM-1 in HIE group was analyzed by correlation analysis with SAS version 6.12. The correlation index of mild HIE group is 0.625 (r=0.625, P<0.01,see Fig1.) whereas that of moderate-severe HIE group is 0.718 (r=0.718, P<0.01, see Fig2.) in acute period. In recovery period, the correlation index of mild HIE group is 0.572 (r=0.572, P<0.01,see Fig3.) whereas that of moderate-severe HIE group is 0.671 (r=0.671, P<0.01,see Fig4.). Conclusions: (1) The serum level of IL-18 and ICAM-1 in HIE patients are higher obviously than non-HIE groups, which shows that they participate in the pathophysiological process of neonate hypoxic ischemic encephalopathy.
