Objective:Alopecia areata(AA) is the common one of the hair disease.However,the definite etiopathology of AA is not complete clearing. Most reference recognized the hereditary,immunology,infection,psycho-nervous factor and endocrinology disturbrance has closed relationship to AA.Nowaday,the lab found,the relationship between viral infection and dermatology pay attention in china and outer-china. Human cytomegalovirus(HCMV) with psoriasis,sextual disorder on mechanism has closed relationship.Recently,some scholars by using electron-microscop found abnormal accumulated small body or granule in cytoplasm of the basal cell or nearly basal cell of damaged skin in AA,presumably its possible with viral infection.HCMV has broad living in the natural world,its natural infection was common.Most of them revealed latent infection.When the body immunologic function is lower,and at this time,the virus was actived and formed active infection,induced cell immunologic and body response.Through the cell factor that produced by cell immunologic and immunolgied cell,such as Interferon-γ(IFN-γ),Tumor necrosis factor-α(TNF-α), Interleukin-2(IL-2) and IL-4 all joined the mechanism.Whether HCMV infection direct induced alopecia or was the AA trigger factor? Recently,its regard reference reported is more less.Our purpose was through these examination the HCMV level in AA patients,to decline the relationship between HCMV and AA,and exploration the relationship of TNF-α and IL-2 level in AA patients with HCMV infection.Materials and Methods: All AA patients were derived from the dermatology department of the Fourth Affiliated Hospital of Hebei Medical University from October, 2002 to October,2003.62 AA patients were selected according to the diagnosis on the basis of history illness ,physical examination and clinical evidence.The AA patients include 34 males and 28 femals;age range from 10 to 56 years(averge 28.6±6.7 years);course from 2 days to 6 years(average 1.9 years);active phrase 38 and stable phrase 24;all patients are alopecia areata.Random selection 90 healthy volunteers as controls; including 52 males and 38 femals, age range from 8 to 60 years(averge 30.4±9.6 years);patients with Hakaru Hashimoto thyroiditis,Diabetes,Hypoadrenocorticism,auto-immunologic hemolytic anemia were excluded.By using Enzyme Linked Immunosorbent Assay(ELISA) to detect the level of HCMV IgM antibodies, TNF-α and IL-2. 4ml blood samples of the 62 AA patients and 90 healthy volunteers were taken from median antebrachial vein,waiting congulation under room-temperature,then centrifugating by 1500/minute for 10 minutes,obtained the serum from the blood,the serum sample were kept in -72℃ refrigerator,then waiting for the collecting to detect at the same time.62 AA patients to detect these threemarker,90 healthy volunteers only to examin HCMV-IgM concentration.All the data were used SPSS10.0 software pack to make statistical-analysis.All the measurement data was denoted by mean±standard deviation and students t test was used to establish significance. Chi-square test was used for analysis of categorical data. Correlation test was used to measur the coefficient of correlation about the correlation data. We took p<0.05 as statistic significance level.Result :1.AA group HCMV-IgM positive and control group HCMV-IgM positive is 41.67% and 18.89%,expectively,the former is more higher than that of the latter.it has significant differences(p=0.004),and the rate of HCMV positive in active phrase is more higher than that of the stable prase(p=0.008).2.Based on HCMV-IgM level to decide AA group for HCMV-IgM positive group and negtive group.The HCMV-IgM positive group was total 26 cases(41.93%),the HCMV-IgM negtive group was total 36 cases(58.07%).There were no statistical differences between both groups about age,sex and clinical course(p>0.05).The materials of two groups is equal,it can be compared.3.The level of TNF-α in HCMV-IgM posive group is 2.90±1.18ng/ml,HCMV-IgM negtive group is 2.29±0.95ng/ml,between tw...
