Objective:The acupoint is the foundation of traditional Chinese medicine. It has not been widely accepted in the world because the existence and mechanisms of acupoint still need to be proved by completely laboratory and clinical experiments. In practice, acupuncturing Tongli (HT-5) and Shangqiu (SP-5) are often used to treat aphasia. However, the physiological mechanisms of this application are still not clear. The purposes of the present study are 1. To characterize the cerebral cortex activation by electric stimulating Tongli and Shangqiu using fMRI, 2. To elucidate the relationship between stimulating Tongli and Shangqiu and the reaction of cerebral speech cortex.Subjects and Methods:30 healthy adults, 16 males and 14 females, are volunteered in this study. Among them 28 right-handed and 2 left-handed, from 19-62 years old. The procedures are as following. Putting poles on the surface of acupoints Tongli and Shangqiu as well as non-acupoint surface, 1cm aside of the acupoints. The stimulation was delivered by using a continuous rectangular wave stimulus at a frequency of 3 Hz and 0.3-0.5mA. A block design was applied in this study. Briefly, each functional run consists of a 60-second stimulation, followed by a 60-second resting for 3 times. Based on the different purpose of the stimulation, 3 groups were designed in the study 1. Control group, stimulating non-acupoint surface. 2.Right-side group, by stimulating right acupoints and 3. Left-side group, by stimulating left acupoints. The instrument used in this experiment is GE 1.5T Signa excite Twinspeed MR. TI WI and fMRI were performed to all subjects in this study. The original figures obtained in the experiment were transferred to the model of ANALYZE. Then SPM99 statistic analysis was performed to get the average activation map. By comparing the map and the Talairach standard spatial cerebral, the location of the activation area were decided. SPSS statistic was applied to compare the diversity of active cortex among the three groups. And t test wasperformed to compare the same cortex in bilateral cerebral. Results:1. Control group:The active areas include left precentral gyrus (voxel 20.79?.79, active degree(T) 2.02?.69), left postcentral gyrus (voxel 74.14?1.73, T 7.32?.45), left occipital lobe (voxel 23.25?.63, T 5.30?.71) and right occipital 1 lobe (voxel 25.79?.56, T5.18?.71).2. Right-side group:The active areas include left precentral gyrus (voxel 10.21?.22, T 5.07?.79), left postcentral gyrus (voxel 7.21?1.48, T 7.80?.82), left superior temporal gyrus (voxel 47.14?.04, T 7.47?.69), left supramarginal gyrus (voxel 74.21?3.64, T 7.80?0.78), left occipital lobe (voxel 39.64?1.20, T 5.45?.86), left frontal pole (voxel 15.71 ?.83, T 5.48?.62), right superior temporal gyrus (voxel 29.29?.95105, T 4.4543?.97), right occipital lobe (voxel 15.50?.73, T 2.76?.81), right supramarginal gyrus (voxel 38.46?0.21, T 5.65?.87) and cingulum(voxel 27.14?0.16, T 5.73?.78).3. Left-side group:The active areas include left postcentral gyrus (voxel 14.61?.99, T5.54?.65), left superior temporal gyrus (voxel 13.36?.33, T 5.56?.64), left occipital lobe (voxel 35.57?.25, T 5 25?.71), right superior temporal gyrus (voxel 18.68?.37 T 6.78?.74), right occipital lobe (voxel 49.71?0.36, T 5.28?79), right precentral gyrus (voxel 28.57?.32, T 5.37?.89), right postcentral gyrus (voxel 55.18?3.57, T 5.91?.87), right supramarginal gyrus(voxel 56.00?.98, T 6.74?.59), rightsuperior frontal gyrus (voxel 11.07?.07, T 5.34?.34) and cinguium (voxel 16.46?.62, T5.49?.71).4. The result of one-way ANOVA among the three groups indicated significance of difference (/><0.05) in right pretcentral gyrus, bilateral postcentral gyrus,bilateral superior temporal gyrus and bilateral supramarginal gyrus.5. The result of paired t-test for dependent samples between the bilateral cerebral active cortex indicated significance of difference (P<0.01) in postcentral gyrus, superior temporal gyrus and supramarginal gyrus in right group and left group.Conclusions:1.
