| Objective : To evaluate the Her-2,β-catenin and E-cadherin expressions in gastric cancer and investigate their relationships with tumor infiltration, metastasis and prognosis of gastric carcinoma.Methods : We gathered 80 specimens of gastric cancer who underwent gastrectomy in An Hui Medical Affilicated Hospital during 1995 to 1998, and 20 specimens biopsied from superficial gastritis were examined as control. Immunohistochemical technique with SP method was used to examine the expressions of Her-2, β-catenin and E-cadherin protein and their relationships with tumor infiltration, metastasis and prognosis. COX proportional regression model was used to define the prognostic factors that determine the survival of the patients.Results : 1. In 80 specimens of gastric cancer, the positive expression rates of Her-2, β-catenin and E-cadherin were 53.75% (43/80), 36.25% (31/80) and 33.75% (27/80), respectively. The expressions of E-cad and β-cat in gastric carcinoma were significantly lower than those in superficial gastritis (P<0.01), and the expression of Her-2 was significantly higher in gastric carcinoma (P<0.01). By Spearman correlate analysis, the expression of Her-2 correlated with those of β-cat (r=-0.239,P<0.01) and E-cad (r=-0.291,P<0.01), negatively, and the significant positive correlation of the expressions between β-cat and E-cad was observed (r=0.409,P<0.01). 2.The x2 test and Fisher's exact test were used to analyze the relationships of Her-2, β-cat and E-cad expressions with the clinicopathological parameters of gastric cancer. There were nosignificant differences between the expressions of Her-2,β-cat, E-cad and sex, age, tumor sizes, differentiation degree, growth of gastric cancer specimens. Her-2 expression was found to be correlative with extent of invasion and distant metastasis (P<0.05), but not to be relative to the lymph node metastasis of gastric cancer (P>0.05). The expressions of β-cat and E-cad significantly correlated with tumor infiltration (P<0.01), lymph node metastasis (P<0.01) and distant metastasis (P<0.01), respectively. 3. There was no difference between Her-2 expression and survival time (P>0.05). But the survival time in patients with positive expressions of β-cat and E-cad was longer than those with negative expressions (P<0.05). 4. By Cox regression model analysis, the expression of E-cad and the infiltration depth were found to be the independent factors for prognosis of gastric cancer patients.Conclusions : 1.The expression of Her-2 is significant higher and the expressions of β-cat and E-cad are significant lower in gastric cancer. The Her-2 expression may reduce the expressions of β-cat and E-cad. 2.The expressions of β-cat and E-cad are significantly associated with infiltration and metastasis respectively, which indicate the important roles in the infiltration and metastasis of gastric cancer. 3.The Her-2 expression is not correlative with prognosis of gastric cancer, but the expressions of B -cat and E-cad are correlative with prognosis of gastric cancer. 4.The E-cad expression and tumor infiltration are found to be the independent factors for prognosis. |
