Primary Clinical Research On Postoperative Speech Disorder In Patients With Cleft Palate

With the early treatment of children with cleft palate,functional speech disorder has increasingly become major part of postoperative speech disorder,therefore,I t's of important significance to clarify the cause of this problem and provide proper treatmental protocol.In this research ,three parts are involved,including research on contributing factors of functional speech disorder ,spectrogram analysis and method of speech therapy.The first study is to clarify the cause of postoperative functional speech disorder,provide evidences for prevention and treatment of functional speech disorder.Method:in the paper.the financial speech disorder after cleft palate surgery was illustrated using descriptive statistics.The potential risk factors of speech disorder were studied with logistic regression modeling.Using data from retrospective study involving pat- ients from follow-up after cleft palate operation.Result:the principal risk factors would include operative ages,parents'education ,proper speech intervene, patients'cleft type and malocclusion .conclusio n: the analytic method in the paper was fit for the study of risk factors of postoperative speech disorder and contribute to clarify the causesof functional speech disorder in cleft palate patients.The second part is to study acoustic features of cleft palate patients with different velopharyngeal conditions,provide a new method for speech evaluati ng of patients with cleft palate during speech therapy. Method:22 patients with velopuaryngeal incompetence(VPI),24 patients with velopharybgealcompetence(VPC) and 15 normal children were analyzed with application of soft ware Speech Analyzer 1.5,the utterance studied were /pa/ /ka/ /si/ /xi/ /zi/ /qi/,then we compared the occurrence of spike and stop gap of different group.Result:There are significant differences among VPI group VPC group and normal group in above acoustic characteristic s,P<0.05. Conclusion: computerized speech analysis techonology can be used to evaluate patients' velopharyngeal conditions, velopharyn- geal is not the only factor which influence cleft palate patients'speech development,the emphasis of speech therapy should be laid on the consonant.The third study is to explore a set of scientific and effective method for Chinese speech therapy in patients with cleft palate ,improve patients'speech intelligibility. Method: 14 patients with functional speech disorder underwent systematic speech therapy and their speech outcomes were analyzed before and after the treatment. Resultsrthe mean intelligibility of the patients was enhanced from 32.14% to 78.29%,there were singnificant differernces between the scores of articulation before and after speech therapy.the features of spectrogram become normal after speech training .Conclusion :our speech training method iseffective,speech training can improve the linguistic development ofthe patients.computer speech analysis is a effective and objectivemethod for evaluating our speech therapy outcome.
