| Class II division 1 malocclusion is common in clinic. It has great unfavorable effects on the oralfacial morphology and function. Early treatment with functional appliance can stimulate dental, occlusal, skeletal and facial development.Objectives: To fabricate domestic Jasper Jumper appliance , research the changes of soft and hard tissues of skull and face with domestic Jasper Jumper appliance treatment to early Class II Division 1 malocclusion cases, and study effects of TMJ with domestic Jasper Jumper appliance to early Class II Division 1 malocclusion cases, investigate the mechanism of Jasper Jumper appliance treatment through the study on the adaptive modification of hard and soft tissues, and TMJ.Mothods: 22 early Class II Division 1 malocclusion patients were treated with the domestic Jasper Jumper appliance for 6-8 months. All patients were made by the cephalometric roentenograms and TMJ lateral tomograms during treatment. The cephalometric roentenograms, TMJlateral tomograms and Helkimo index were used to evaluate the adaptive modification of hard and soft tissues, and TMJ Pre- and Post-treatment. Results:1. Fabrication of domestic Jasper Jumper appliance Domestic Jasper Jumper appliance was fabricatedsuccessfully with Xihu Bio-Technological Company. Used Jasper Jumper appliance in growing children with Angle class II division 1 malocclusion and mandibular retrusion, the marked curative effects were gained.2. Changes of hard and soft tissue after treating with domestic Jasper Jumper appliance in growing children with Angle class II division 1 malocclusion and mandibular retrusion(1) After 6-8 months (average 7 months) of Jasper Jumper appliance therapy, it inhibited maxillary growth, which resulted in 0.69 reduction of SNA and 0.42mm reduction of the distance from A to OLP plane. Class II malocclusal relationships were changed to Class I. Class II molar relationship and Overjet were corrected average 6. 92mm and 7. 02mm.(2) Mandible changed significently. SNB degree, mandibular ramas height, mandibular length, mandibular body length and the distance from Pg to OLP plane were increased 3. 12, 3. 57mm, 6. 66mm, 4. 87mm and 3. 5mm respectively. The amounts of increase were beyond the growth changes in nature significant in clinic.(3) Sagital changes appeared with the decrease of ANB markedly about 3. 71. Vertical changes came with the increase of anterior facial height (N~Me), lower anterior facial height (ANS-Me), posterior facial height (S-Go), lower posterior facial height (Ar-Go) 6. 03mm, 4. 33mm, 5. 42mm, 3. 64mm respectively. It showed that Jasper Jumper appliance therapy stimulated the growth of lower facial hard tissue and corrected discrepancy between sagital and vertical skeletal relationship.(4) Jasper Jumper appliance therapy brought about a 2. 85"reduction of the facial soft tissue profile convexity. Upper lip base angular, upper lip angular remained unchanged before and after treatment. Lower lip base angular, lower lip angular, soft tissure mental angular increased 2. 85, 11. 45, 4. 44 respectively and H angular, upper lower lip base angular and mentalabial sulcus depth decreased 3. 7, 2, 3. 22mm respectively. It showed that Jasper Jumper appliance treatment stimulated growth of lower facial soft tissue and was beneficial to reduce facial convexity. However, decrease of mentalabial sulcus thickness suggested the modification of soft tissue fell behind skeletal growth.3. Changes of TMJ after treating with domestic Jasper Jumper applianceAt the beginning of Jasper Jumper appliance therapy, temporary mild TMD symptom appeared in 4 patients and Helkimo index increased. After therapy, TMD symtom wasrelieved.
