Objective To study the surgical treatment patterns of dissecting aneurysm of thoracic aorta.Methods During the May 1997 to January 2003, 21 patients with dissecting aneurysm of thoracic aorta, including 14 cases of male and 9 cases of female, mean age 51.1?2.9 years, range 30 to 78 years, underwent surgical treatment. Based on DeBakey's classification, 9 cases were in type I and 7 cases as type II, 5 cases of type III. The procedures were performed including 12 Bentall operation, 4 implantation of intraluminal prosthesis for ascending and arch aorta, 5 endoluminal stent graft implantation for descending aorta. Myocardial protection methods were sequence coronary artery ostium perfusion, or sequence perfusion and continuous retrograde coronary sinus perfusion with cold oxygenated blood cardioplegia. 2 cases underwent deep hypothermia circulatory arrest (DHCA) with retrograde cerebral perfusion (RCP) and others were moderate hypothermia cardiopulmonary bypass.Result One patient died of multiorgans failure the third postoperative day, one discharged the first postoperative day, others recovered and were discharged.Conclusion The dissecting aneurysm of thoracic aorta should be diagnosised in time. Satisfactory result can be obtained by comprehensive preoperated evaluation, proper operation, skilled surgery and good organs preservation.
