Objective: To study the mRNA expression of insulin receptor substrate-1 (IRS -l),glucose transporter-4(Glut4) and resistin in omental adipose tissue of metablism syndrome(MS) patients and their relation with waist hip ratio(WHR), body weight index(BMI),insulin resistance index(HOMA-IR),fast blood glucose(FBG),fast insulin (FINS), triglyceride (TG) and blood pressure.Method: The mRNA expression level of IRS-l,Glut4 and resistin in omental adipose tissue of 28 cases with MS and 25 cases of controls was measured by one-step semi-quantitative RT-PCR.Comparisons among groups were made by unpaired t-test ,One-Way ANOVA or nonparametric Kruskal-Wallis Test. The relationship between or within various variables was analyzed by Pearson correlations or nonparametric Spearman rank correlations and stepwise regression analysis.Result: (1) The mRNA expression of IRS-1 and Glut4 in omental adipose tissue was significantly decreased in patients with MS than that in controls (0.276±0.065 vs 0.529±0.079,P<0.001; 0.349±0.085 vs0.595±0.094, P<0.001). (2) IRS-1 and Glut4 mRNA expression in omental adipose tissue from type 2 diabetic patients was significantly lower than that of non- diabetic MS patients ( 0.232±0.044 vs 0.314±0.057, P<0.001; 0.294±0.027 vs 0.396±0.091 , P = 0.001) .(3)In Two Groups,the mRNA ex- pression level of IRS-1 was positively correlated with that of Glut4 respectively(r=0.661 ,P<0.001; r=0.621 ,P=0.001). (4) Their was significant correlation within WHR ,HOMA-IR and IRS-1, Glut4 mRNA. (5) No difference of resistin mRNA expression was found among type 2 diabetes, non-diabetic MS patients and controls (P=0.228) ,and no correlation between resistin mRNA expression and BMI, WHR, HOMA-IR ,FBG, FINS, TG and blood pressure.Conclution: MS patients showed lower gene expression of IRS-1 and Glut4 in omental adipose tissue than that of control group,as far as type 2 diabetic patients were concerned ,above results were obviosly decreased.The level of IRS-1 andGlut4 mRNA expression in visceral adipose tissue and WHR seemed to be important parameters of insulin resistance.There seemed no relationship between resistin mRNA expression in omental adipose tissue and MS.
