| Objective: To evaluate relationship between HLA and IgA nephropathy and it's clinical presentation,pathology, We study HLA-DR gene susceptibility to IgA nephropathy in Guangxi Zhuang and Han population. Methods: HLA-DR4 and HLA-DR12 genes in 87 cases of IgA nephropathy patients and 110 normal controls were investigated by Polymerse Chain Reaction-Sequence Specific Primers(PCR-SSP) technique. Sequences of HLA-DR4/HLA-DR12 genes of 4 samples were analysed by direct sequencing of PCR products.The results of sequencing were compared with their corresponding sequence data. Results: In Zhuang population ,the frequency of HLA-DR4 gene in IgA nephropathy patients was significantly higher than that in normal controls(16%vs5%,P<0.05,RR=4.24);and proteinuria is more frequent in HLA-DR4-position patients than that in HLA-DR4-negative patients (87.5%vs35.29%,P<0.05,RR=12.83).In Han population,the frequency of HLA-DR12 gene in IgA nephropathy patients was significantly higher than that in normal controls(20.97%vs10%,P<0.01,R=2.89);and proteinuria is more frequent in HLA-DR12-position patients than that in HLA-DR12-negative patients(84.62%vs47.22%,P<0.01,RR=6.15); renal failure is more frequent in HLA-DR4- negative patients than that in HLA-DR4-position patients(49.02%vs9.09%,P<0.05,RR=9.62). Conclusions: HLA-DR4 gene may be susceptible allele in patients with IgA nephropathy in Guangxi Zhuang population,and HLA-DR4 gene is associated with clinical presentation; HLA-DR12 gene may be susceptible allele in patients with IgA nephropathy in Guangxi Han population,and HLA-DR4 and HLA-DR12 genes are associated with clinical presentation. |
