Evaluation Of The 5-year Clinic Effect By Comparing Two Different Types Of IUD

OBJECTIVES: From the analyze and comparing two different types of IUD of the 5-year clinic effect and its affection factors and the affection to women's health, afford theory and practice base to control reproduction.METHODS AND METERIAL: Historical cohort study will be used. The regularly menstruating women at the age of 20 to 40 in the regions of Pinyin and Taiga in Shania, who are willing to use FRCu200 or MLCu375SLIUD from Jan. 1996 to Jul. 1996, have participated in the study. According to the purpose the research, the items we have studied are as follows: age, occupation, labor intensity, labor posture, gestational history, types of IUD, side response, position and vertical length of uterus, KB, blood pressure, situation of pelvic and genitals after IUD insertion. The items for evaluating the effects are as follows: continuation rates, slippage rates, gestation rate with IUD, removals rates for leading or pain. The five years subsequently data of using FRand ML Cu-IUD will be analyzed with t-test and chi-square test and log-rank test and Cox Regression will be used.RESULTS: Continuation rates in five years of FRCu200IUD and MLCu375SLIUD are 77.8% and 79.8% respectively, the acreage value is 78.8%, and there is no statistically difference by log rank Test ,x2=0.12, P=0.7291,Side effects rates are 30.8% and 33.7% respectively, and there is no statistically difference by log rank Test x2=0.14, P=0.706 . The main factors to affect slippage of FR Cu200C and ML Cu 375SL are unexpected gestation, slippage, removals for blooding or pain, removals for personal reason, removals for medical treatment, removals for gestation plan. Stop rates in five years of FR are 2.2%, 15.4%, 4.4%, 2.2%, 1.1%, 6.6% respectively, stop rates in five years of ML are 2.1%, 9.5%, 8.4%, 3.2%, 1.1%, 5.3% respectively for the six reasons above, and there is no statistically difference by Chi-square Test (P>0.05). Cox Regression has been used to analyzed the 13 possible factors affecting to stop use of IUD, the results show that main factors to affect to stop use of IUD are age, labor intensity, and habitual abortion. The more of age, labor intensity, and habitualabortion, the more of to stop use . All the women participated in the study have no anemia, high blood pressure, pelvic and genitals infection.CONCLUSIONS: From the comparison of five years clinic effects, FRCu200IUD and MLCu375SLIUD all have high contraceptive affection. And they are all effective in contraception and safe to women's health. The results of Cox Regression show that the regularly menstruating women, who are old or have heavy labor intensity and habitual abortion history, should not use FRCu200IUD and MLCu375SLIUD.
