Objective Rheumatoid arthritis(RA) is a complex autoimmune disease .It is characterized by the activation of peripheral blood lymphocytes, chronic inflammation in the joints, progressive destruction of cartilage and bone , functional disability .Increasing evidence shows that nuclear factor-kappaB(NF-kB) is activated in RA, NF-kB play a key role in the pathogenesis of RA .Inhibition of NF-kB can be a novel target to treatment RA,and a new way to screen anti-rheumatic drugs.Sinomenine (Sin.)is extract of Sinomeniwn acutum which is a traditional Chinese anti-rheumatic herb.but the anti-rheumatic mechanism is uncertain.This study was designed to illuminate the the activity of NF-kB in peripheral blood mononuclear cells (PBMC) from RA patients and normal people and the effects of Sin. on the activity of NF-K B by PBMC from patients with RA, explain the molecular anti-rheumatic mechanism of Sin-Method 10 active RA patients fufiled with American College of Rheumatology criteria and 10 healthy donors were involved in the study , PBMC was isolated , and cultured in RPMI-1640 with and without PHA and/ or Sin. , PBMC that cultured in RPMI-1640 were as controls, incubated in 5% CO2 cabinet for 6 h, collected cultured PBMC, extracted nuclear protein, NF-k B was detected by ELSIA. A value was measured at A450nm by spectrophotometerResults 1. Elevated activity of NF-kB was found in PBMC from RA patients compared to the normal controls(P<0.001).2. Elevated activity of NF-kB was found in normal PBMC with PHA compared to the normal controls(P<0.001).3. Elevated activity of NF-k B was found in PBMC from RA patients with PHA compared to the RA patients controls(P<0.001). 4.Decreased activity of NF-kB was found in normal PBMC with Sin compared to the normal controls(P<0.001).5. Decreased activity of NF-kB was found innormal PBMC with Sin and PHA compared to the normal PBMC with PHA controls (P<0.001),There are significance among the different sin concentrations. (P<0.001), Dosage is dependent.6. Decreased activity of NF-kB was found in PBMC from RA patients with Sin compared to the RA patients controls(P<0.001).7. Decreased activity of NF-kB was found in PBMC from RA patients with Sin and PHA compared to the PBMC from RA patients with PHA controls(P<0.001),There are significance among the different sin concentrations. (P<0.001).Dosage is dependent.Conclusions 1. There are abnormal elevated activity of NF-kB in PBMC from RA patients ,there are basic activity of NF-kB in normal PBMC.2.Sin inhibits the activity of NF-kB in PBMC from normal and RA stimulated with and without PHA.3. The anti-rheumatic action of Sin is partly related with the inhibition of the PBMC NF-kB P65 pathway .
