| OBJECT IVE: To study the accuracy and practicability of that bone age assess to diagnose idiopathic precocious puberty.OBJECT AND METHODS: The study was made up of 55 girls with idiopathic precocious puberty, age 3.2-9.9(7.55±1.90)years, before the 8-years-old breast had developed 36 patients, age 1.5-7.9(6.07 ± 1.72)years; before the 10-years-old have had menophania 20 cases, age 5.0-9.75 (8.75 ± 1.26) years. The control have 4 groups: (1) after 10-years-old girls have menophania 14 cases, age (11.45±1.05) years; (2) age 8-10 years girls with body weight was light 25cases(9.2 ± 0.67years); (3) age 8-10 years girls with body weight was normal 32 cases (9.11 ±0.74years); (4)age 8-10 years girls with body was over-weight 12 case (9.61 ± 0.66years). retrospectively analysised these first x-ray-film of left hand and wrist in first visit, single-blind analysis by TW2 methods to assess bone age; bone age have RUS , carpale and 20 bones 3 systemes, one of them have 5 decision threshold. RESULTS: (1) There were 4 points of both of sensitivity and specificity were relatively higher : (1)RUS of " > P90" (Sen=0.836, Spe=0.916), (2)carpale of ">P90" (Sen=0.746, Spe=0.916), (3) 20 bone of ">P90" (Sen=0.746, Spe=0.964) and?">P75" (Sen=0.982, Spe=0.783);(2) Area under a receiver operator characteristic (ROC) curve: RUS of A13=0.939 ± 0.019 (95%CI: 0.902-0.977), carpale of A7=0.899 ± 0.028 (95%CI:0.845~0.954), 20 bone of A20=0.958 ± 0.014 (95%CI: 0.930-0.986), F=2.03, P=0.13; (3) Assessment of agreement: within-observer reliability was 89.28%, between-observer reliability was 80.3%; Kappa=0.68 (w=6.87,JP<0.01). CONCLUSI ON: The bone age of RUS and 20 bones were high accuracy for that diagnosed idiopathic precocious puberty, we confirmed decision threshold of RUS of " > P90" was the best point, after synthetically analysed sensitivity and specificity.
