Objective: To evaluate the distribute of prehospital time in thrombolysis in the patients with acute myocardial infarction with ST segment elevation, and to study the prehospital factors which cause delay.Methods: 147 eligible patients were interviewed by telephone call, andhistory of illness was inquired. The delay in the prehospital stage was evaluated and analyzed respectively according to whether the thrombolysis had been carried out or not. The prehospital factors which resulted in the delay were studied.Results: The mean time consumed in the prehospital stage was 240minutes, 75% occurred in the period of making a decision to seek medical assistance. Delay has been elongated by self-management after heart attack, first consultation to community clinic, and symptom presentation at home. Longer delay is associated with the following factors as prior history of coronary heart disease (CHD), no recognition of heart attack and ascribing abnormal syndromes to no heart disease. 41 patients were thrombolysed.Conclusions: The prehospital time accords with the tune limit wordlyon the whole in some patients with acute myocardial infarction with ST segment elevation in west china. We may improve greatly the chance of thrombolysis if we can shorten time from hospital presentation to thromholysis. The current major factors influencing prehospital time are initial action, first medical consultation site, environmental factors, prior history of CHD and initial interpretation to symptoms.
