The pathogenesis of ulcerative colitis is still unknown, a large amount of studies have shown that the synergism among genetic susceptibility, intestinal flora and autoimmunity may lead important roles to the occurrence and preservation of this disease. Although an identified bacterium is all along not found in the intestine of ulcerative colitis patients, the lose balance between residential flora are popularized by most of researchers. Recently, glucocorticoid, salazosulfamide and immunosuppressive drugs are still cardinally used to treat patients, however, the side-effects of these drugs are obviously such as relapse after drugs withdrawal. Karen et al tried to use bifidobacterium et al three alive bacteria to treat patients who are resistant to 5-aminosalicylic acid, and the results have been identified. The study investigated the roles of probiotics in intestinal mucosa of ulcerative colitis patients through analysis of residential bacteria, detection the expression of protein and observation the activation of NF-KB. The aims of the study are to research the pathogenesis of UC and prevent the recurrence of UC through further investigating the relationship between intestinal flora, activation and expression of proinflammatory cytokines in intestinal epithelial cells of UC patients, and to offer theoretical basis for effective therapy. Methods.-1. Using swift-flora smear to detect the stool of 41 patients who were diagnosised by gastroenterologists and analyze the mRNA expression of proinflammatory cytokines and the degree of inflammation in intestinal epithelial cells according to the degree of dysbacteriosis.2. 1. Thirty serious patients in clinical and endoscopic remission by sulphasalazine (SASP) and glucocorticoid were randomized to receive eitherBifid Triple Viable capsule (BIFICO), 1.26g/day, or an identical placebo (starch) for 8 weeks.2. Analysis of flora: detecting 10 residential bacteria of human beings' intestine through the existing method of our department before and after treatment with BIFICO.3. The expressions of TNF-alpha, IL-1beta and IL-8 mRNA in the intestinal mucosa epithelial cells of UC patients were detected by a semi-quantatitive assay RT-PCR.4. The activation of NF-кB in the epithelial cells was evaluated by electrophoretic mobility shift assay (EMSA).5. The aggregations of NF-кB in nucleus and IкB in plasma were evaluated by western blot.Results:1. With the increase of flora disorder, the mRNA expressions of proinflammatory cytokines in epithelial cells have been up-regulated.2. Three patients (20%) in the BIFICO group had relapsed within the 2-month follow-up period, compared with 14 (93.3%) in the placebo group3. Fecal bacteria number before treatment of these groups was decreased after taking sulphasalazine and glucocorticoid but had no significant difference. After treatment, fecal specimens of the patients who took BIFICO were obviously different from the patients who were treated by placebo. Gram-positive Bacillus and Enterococcus were significantly more than that of the control group such as Enterococcus, Bacteroides, Bifidobacterium (P<0.05), but Saccharomyces were obviously less than the control one (P<0.05); In the control group, Bifidobacterium and Lactobacilluswere were less than the treatment group (P<0.05), Enterobactera increased significantly (P<0.05).4. The proinflammatory cytokines mRNA expressions of intestinal epithelial cells were restrained in BIFICO treatment group (P<0.05). On the contrary, in control group, TNF-ot, IL-1beta, IL-8mRNA expressions were elevated more significantly than remission period after taking placebo(P<0.05).5. The results of EMS A illustrated that activation of NF-кB binding DNA was down-regulated after taking BIFICO, which was significantly discrepant with control group (P<0.05).6.
