Objective: To evaluate the efficacy and feasibility of all-trans retinoic acid drug delivery system(atRA DOS) for prevention of posterior capsule opacification(PCO) in rabbits. Methods: (1) 34 New Zealand white rabbits were divided randomly into normal group, control group, empty DDS(EM DDS) implanted group, 75ug atRA DDS implanted group and J50ug atRA DOS implanted group .They all received clear lens phacoemulsification on both eyes, except normal group. Then the three groups were implanted respectively empty DDS(containing no atRA), 75ug atRA DDS, 150ug atRA DDS in posterior chamber. The control group was implanted nothing. (2) At 2, 7, 15 and 30 days postoperatively, atRA concentration in aqueous humor was detected . The effect of the expression of proliferating cells nuclear antigen (PCNA) in every group was examined by SABC immunohistochemical method. (3) Electron microscopy examination on changing of posterior capsule of lens was observed in every group. (4) Histopathological examination on local ocular tissues were performed to evaluate the toxic and side effects of atRA. Results: (1) The mean atRA concentration in aqueous humor of every group was 3.07 ± 0.15 ng.ml-1 ,3. 12±0. 45ng.ml-1, 3. 10±0. 31ng. ml-1, 80. 62 ± 6. 29ng. ml-1, 122.07 ± 4.89ng. ml-1.The atRA concentration of the 75ug atRA DDS implanted group and the 150ug atRA DDS implanted group can maintain stably in 7 days and 15 days postoperatively. (2) Compared with the control group and EM DDS implanted group, the expression of PCNA was lower in 75ug atRA DDS implanted group and 150ug atRA DDS implanted group, and there was statistical difference among them(P<0. 0.5). (3) The residual lens epithelial cells(RLECs) of control group and EM DDS implanted group migrated obviously than the two atRA DDS implanted groups by electron microscopy examination. (4) However, the difference of damage to corneal endothelial cells among them was not evident, and thestructure of iris,ciliary body, retina were intact, without obvious toxic reaction. Conclusions: The atRA I)I)S could maintain high concentration of atRA in aqueous humor for a long period of time after the operation of cataract extraction and had effective inhibition on proliferation and migration of RLECs, but little toxicity.
