To evaluate ]H Magnetic Resonance Spectroscopy ( HMRS) in the study of schizophrenia.MethordsTwenty - six cases diagnosed as schizophrenia clinically were selected to perform 2.0 Elscint superconductive MRI and 'HMRS examination. In the meantime, twelve healthy volunteers matched for sex, age, educational years was recruited as controls also underwent MRI and 'HMRS. SE sequence were used for routine examination; Point resolved spectroscopy sequence (TR 2000msec, TE 45msec)was required for HMRS. The metabolites in the spectra includes; N -acetylaspartate(NAA) ,choline compounds (Cho) ,creatine compounds ( Cr) , myo - inositol ( MI) , Lactate ( Lac ), glutamate and glutamine ( Glu - Gln) and so on.Results( 1 ) 'HMRS manifestation of normal persons (12 cases ) : Thepeaks of NAA/Cr ,Cho were elevated; and those of MI were slightly decreased; while peaks of Glu - Gin arid Lac were flattened or unde-tectable. The amplitudes in high - to - low order were as follows; NAA,Cr,Cho ,MI ,Glu - Gln and Lac.(2) 'HMRS manifestation of the patients with schizophrenia: There are significant difference of manifestation of 1HMRS between patients with schizophrenia and normal persons. There are statistic significance of NAA/Gr,NAA/Cho and Glu - Gln/Cr in dorsolateral pre-frontal cortex between normal persons and patients with schizophrenia, among them, the statistic difference of NAA/Cr is most apparent. NAA/Cr and Glu - Gln/Cr of dorsolateral prefrontal cortex are closely related with cognitive deficits and the total negative syndromes scales of BPRS. There are statistic significance of NAA/Cr in hippocampus of patients with schizophrenia.( 3) MRI manifestation was abnormal in patients with schizophrenia.DiscussionIn order to get rid of the effects of antipsychotics and long term , this study applied the first - time onset patients whose were not exceeded two years . The patients were not mediated with neuroleptics,too. The results show the decrease of NAA in dorsolateral prefrontal cortex and hippocampus which mean neurons deficit and dysfunction at the time of onset.The results of this study show that the cognitive deficits of WCST and the total negative syndromes scales of BPRS were related to the decrease of NAA in dorsolateral prefrontal cortex. It demonstrate the dysfunction of prefrontal cortex is a very important neuropathology injury in schizophrenic and related to cognitive deficits and some nega-tive syndromes.The study about the decreased of NMDA - glutamatergic function of schizophrenia are paid more attention to in the present time . Therefore , we advance assessing the value of Glu - Gin and Glu - Gln/Cr with the first time . The results of this study show the dysfunction of glutamate in dorsolateral prefrontal cortex and the relationship between the decreased of NMDA - glutamatergic function and cognitive deficits and some negative syndromes.1HMRS manifestation of the patients with schizophrenia support some hypothesis of pathophsiological mechanism of schizophrenia.Conclution(1 ) HMRS manifestation of the patients with schizophrenia; There are significant difference of manifestation of ' HMRS between patients with schizophrenia and normal persons. The decrease of NAA is a sensitive index in the judgment of neuropathological injury of patients with schizophrenia and is more sensitive than MRI. There are important value to use 'HMRS in study pathophsiological mechanism of schizophrenia.( 2) There are neurons deficit and dysfunction in dorsolateral prefrontal cortex of schizophrenia. Both of them are related with cognitive deficits and the total negative syndromes scales of BPRS.(3)There are neurons deficit and dysfunction in hippocampus of schizophrenia.(4) MRI manifestation was abnormal in patients with schizophrenia.
