| PrefaceSPN is the most common disease in pulmonary radiological examination and is very difficult to diagnosis correctly. With the development of radiological technology and the applicant of Multi -slice CT, more and more micro structure of pulmonary lesion and its surroundings can be clearly seen in images. Especially from 1990s , the vessel convergence sign has been one of the focuses of this field.Related research on vessel convergence sign now are limited to lung cancer and the result explain that this sign is postively related with nodular malignancy. While other researches also demonstrate that vessel convergence sign also can be seen in SPN of inflammation and tuberculosis. To further understand the sign , we use MSCT to evaluate it from vessel makeup and morphology ; aimed to find the characters of SPN in all kinds of pulmonary diseases and its formation mechanism ; improve the diagnostic accuracy of SPN lesions.Materials and Methods81 patients were included in our study according to SPN stand-ard. Among them , 41 were lung cancer , 23 were inflammation, 11 were tuberculosis and 6 were other bengin diseases. All the cases were examed with Multi - slice CT before and after medicine theraphy or before surgery, part of them scaned with thin slice after being contrasted.Toshiba Aqullion Multi - slice Spiral - CT machine were used. We scaned the whole lung from tip to fundus routinely at first and then scaned the area from the focus to hilus with thin slice. We used Toshiba ALATOVTEW image - disposal software and reconstructed the orginal images with MPR function at workstation. We continuelly screen the images to observe the construction of the parafocal vessels from different directions till hilus and decided the composition and the property of these vessels.Two rich - experienced doctors examed each case to confirm the existence and the composition of the vessel convergence sign and diffe-riated them as : Type I including both pulmonary artery and vien. Type II including only pulmonary artery. Type H including only pulmonary vein. According to their manifestation , the signs were also differated as three kinds : 1) single widen vessel of the focus' hilum side passes through or truncated in or was tractated to the focus. 2) two or three widen vessels of the focus' hilum side were tractated to or pass through or truncated in the focus. 3 ) many widen and tortouse vessels including both hilum side and pleura side converg aroud the focus.We calculated the positive rate of vessel convergence sign in different kinds of diseases at first , and then compared the differiation of the type and manefestation of the sign in different diseases . We use X2 test to determine whether the difference has statistical significance.Result1. Positive rate of vessel convergence sign in different SPN diseases.34 lung cancer patients are positive (82.9% ) , while 13 inflammation (56. 5%) , 9 tuberculosis (81. 8%) and 1 focal fibrosis (16. 7% ) are positive too. Analysis ; the differiation between cancer and tuberculosis isn't significant (p >0.05) , while the differiation between cancer and inflammation is significant (p <0. 025) . the differiation between inflammation and tuberculosis doesn't have statistical signification.2. The comparation of vessel composition of vessel convergence sign in different SPN diseases.The composition of vessel convergence sign in cancer and inflammation are different and statistical significant . Further statistication show ; For Type I , inflammation has higher rate (50% ) , while cancer is 31.2% , and the differiation is significant (p <0.0005 ) ; For TypelH , cancer has higher rate -(82. 6% ) , while inflammation is 8. 7% , and the differiation is significant (p <0.0125) ; For Type II , the differiation doesn't have statistical significance.3.
