It is known that the histological peculiarity or the pathological grades of the glioma are the most important factors in evaluating the prognosis of the patients. However, because of their limits, the result evaluating with pathological grades is not always coincidence with the malignancy or the biological behavior of gliomas. So evaluating the malignancy of the glioma accurately not only could predict the prognosis exactly, but also is important to chose correct therapy way. We do a quantitative test of 99m Tc-MIBI uptake index and Ki-67 labeled index to study the relationship between metabolism and proliferation of tumor, the give a more clear mechanism of uptake of 99m Tc-MIBI in tumor; at the same time we do some search between pathological specifity and their staging of tumor to evaluate the relationship between 99m Tc-MIBI uptake index and Ki-67 and tumor pathological staging and the significance in glial biological behavior.METHODS31 cases of primary glioma patients, divided in two groups according to the newest WHO classification of intracranial tumors. 16 cases is low degree glioma ( degree I - II ) , 15 cases high degree glioma ( degree III - IV ) ; all samples are from surgical resection andlabeled after surgery. The evalua--tion of tumor local uptake uses asemi - quantitative analysis about foci area of interest and contralateral corresponding area of interest, and we performed an immunohisto-chemical technique to label Ki-67 protein. After all work finished, we randomly counted 1000 cells in each sample to calculate the number of positive cells; then we use bilateral unpaired Students t test to analyze the data of high and low degree glioma groups to determine the relation - ship between 99m Tc-MIBI uptake index and Ki-67 labeled index. All tests were applied with t test.RESULTRelationship between 99mTc - MIBI uptake and histological degree of tumor: uptake value of low degree group is 0. 8 - 1.7, average 1.27 ± 0.12; in high degree group the value is 1.9 - 6.6, average 3. 76 ± 0.65, there is a significant difference in two groups( p < 0.01). Relationship between Ki-67 labeled index and tumor histological degree : uptake value of low degree group is 1.5 - 16.7, average 5.96 ± 1.96, and in high degree group is 15.4 -66. 7, average 31. 8 ±5. 60. There is also a significant difference between two groups (p <0. 01). Analysis between 99m Tc-MIB uptake index and Ki-67 labeled index; correlation coefficient r =0. 84, t test show p <0.01, the 2 indices are correlative.CONCLUSIONThe mechanism of uptake of 99m Tc-MIBI in brain glioma may have relation to the destruction of BBB and cerebral blood stream, andhave consanguineous relation with tumor cell metabolism activity. 99m Tc-MIBI uptake index and Ki-67 labeled index are positive correlative, and two indices have the accordant histological degree. We can say the two indices have obvious mutual complementtarities in evaluation of pathological staging and biological behavior of tumor.
