The Determination And Clinical Significance Of TGF-β Level In Children With MsPGN

Transforming growth factor-β1 (TGF-β1) is a multifunctional cytokine.It has been reported that TGF-β1 plays an improtant roles in the pathogenesis of mesangial proliferation and glomerular screlosis. The plasma,serum and urinary TGF-(1 levels were measured in 138 children including 30 with MsPGN ,30 with ML, 24 with IgAN,24 with FSGS,and with 30 normal children by sensitive enzyme-linked immunosorbent assay(ELISA),and the correlation of TGF-β1 level with clinical features and pathologic types was analysed.The purpose of the study on the determination of the TGF-β1 level was to judge the pathomorphology of MsPGN and monitor the severity of illness. Results of the study indicated that: 1.The sensitivety and specificity of the TGF-β1 level in the plasma,serum and urinary samples were the best,the next,and the third,respectively.The TGF-β level in plasma and urinary samples had significant correlation and consistency. 2.The TGF-β1 level of plasma,serum and urinary samples in controls,ML,MsPGN,IgAN,FSGS groups was gradually increased.The TGF-β1 level of patients with MsPGN was higher than in controls(P＜0.05). The levels of TGF-β1 between MsPGN and IgA or FSGS had more significant difference in Plasma and urinary (P＜0.01).Serum level of TGF-β1 had also significant difference(P＜0.05). The TGF-β1 level was correlated with inflammatory cells infiltration in interstitial tissue of kidney with MsPGN.The level of TGF-β1 with infiltration group was higher than that without infiltration group( P＜0.05). The TGF-β1 level in the serious proteinuria with MsPGN had significant difference as compared with mild or moderate proteinuria group(P＜0.05).Conclusion: 1. Urinary sample is a preferable index to determine TGF-β1 level.Its sensitivety and specificity are better than that of plasma or serum. 2. It is obvious that various pathologic types have different plasma,serum and urinary TGF-β1 level.Plasma,serum,and urinary TGF-β1 values in children with FSGS and IgA were significantly higher than those children with MsPGN,which were higher than controls.Children with ML were almost normal. The TGF-β1 level was significantly correlated with the degrees of mesangial proliferation or inflammatory cells infiltration in interstitial tissue of kidney or with the severity of proteinuria. 3. Measurement of TGF-β1 level might indrectly reflect various types of pathomorphology and the severity of renal diseases.It might be helpful in judging clinical pathomorphology of MsPGN and dynamic observing the severity and process of renal diseases.
