ForewordIn recent years, marked advances have been made in the techniques of repairing tendon injury, but the treatment of finger flexor tendon injury is still one of the most difficult and important problems in orthopedic field. The main problem in tendon repair is adhesion between the tendon and surrounding tissue. To prevent the adhesion and achieve tendon union, we developed adhesion shields for tendon repair using collagenase for injection, and carried out an implant experiment using the deep flexor tendon of the third toe of Leghorn hens.Experimental material1. Experimental animal; 64 adult Leghorn hens (weigh range 1. 1-1. 9kg). All of the hens were divided into two groups randomly. Each group had 32 leghorn hens.2. Experimental drugs: collagense for injection ( each vial contains 1200 units of collagenase ) dissolved with 2ml 0. 9% Natrium chloride.3. Experimental Instruments: light microscope, slice machine.Experimental method;64 adult Leghorn hens were used in the experiment. All of thehens were randomly divided into two groups, each had 32 hens. The Leghorn hens were anaesthesiaed by ketamine, cut flexor profundus tendons at zone II in the third toes of the left leg, and repaired the incision with a modified Kessler type suture (5/0 nylon). 0. 15ml of collagense for injection were placed around the repair site after flexor tendon repair , while 0. 15 ml of injection natrium chloride solution were placed around the repair site in the control group, respectively. Skin incisions were closed and the legs were fixed in slight palmar flexion for 6 weeks . They were feed alone under the same condition and examined at 2,4,6,8 week.1. Biomechanical mensuration:1) The mobility of the tendons: range of tendon movement when appended object weighed 2. 94N.2) Anti - tension strength meansuration: re - rupture was observed at every period after the operation.2. Naked eye observation;Irrelevant surgeons were invited to dissect hens in each group under operational microscope. Evaluation was carried out according to Tangjinbo' s standard.3. The light microscopy observation;Flexor tendon including muscles were completely taken out from every animal, then fixed using 10% formaldehyde for 48hours. 5% nitric acid was used to take off the calcium for 2 weeks. The flexor tendon was divided into 3 segment equally, dehydrated and embeded with wax. Each segment was sliced into 2 pieces, then observed under light microscope after HE dyeing.4. Statistical analysis;Quanlitative data were analysed by mean ±standard deviation, ttest was used to compared the difference between experimental group and the control group. P < 0. 05 was thought as statistical significance.Result1. The length meansuration of tendon after movement; Tab. 1 The length of tendon after movement( cm) ( x @ s)n=8 *:p<0.05 **:p<0.012. Anti - tension strength meansuration of flexor tendon: Tab. 2 Anti - tension strength meansuration of the flexortendon(g) (x @s)n = 5 : p<0.05 **: p<0.013. Observation of tendon adhesion.-Evaluation was carried out according to modified TANGJB ' s standard.4. Light microscope observation :1)2 weeks after operation;Control group : Outside the deep flexor tendon was filled with granulation tissue. Fibroblast was active.Experimental group;The medicine could be seen outside tendon. Fibroblast was not as active as that of control group. Collagen was sparse. The deep flexor tendon was filled with granulation tissue. Neutral white cell reaction was obvious.2)4 weeks after operation;Control group : Most of the fibroblast changed to fiber cell . Vas-cularity decreased.Experimental group;No medicine composition was seen. Inflammatory reaction was alleviated . The adhesion of the deep flexor tendon was more than that of 2 weeks . Matured fiber cell was still less than that of control group. The density of collagen was low.3)6-8 weeks after operation;Contro...
