| Open colonic wound which is soaked by seawater is often occurred in the wet and hot environment in southern war zone in the course of navel battle and working in the sea. Generally open colonic wound soaked by seawater should be treated with second operation of colostomy. We tried to treat this syndrome with primary operation of drainage by double tubes up and down the anastomotic stoma through anus in this experiment and expect to provide laboratory data for clinic application of the therapy in web and hot southern war zone.Materials and methods1. Animals and groups32 adult hybrid dogs were randomly divided into four groups:test and control groups with fire-arm injury and test and control groups with puncture wound. In 3 and 7 days after operation,four dogs of every group were examined respectively.2. Environmental conditionsIn laboratory,temperature of dry ball was kept in the range of (31-35)C and temperature of wet ball (28-31)C. Relative humidity was less than (65-88)%.3. Making of animal modelBuild animal model with open fire-arm or puncture wound in colon which was soaked by seawater. Then treat them by primary operation and put tubes up and down stoma to drain through anus.4. Examination indexes(1) Drain of the double tubes after operation(2) Mortality of postoperation.(3) Change of pressure in colon up and down stoma.(4) pre- and post-operative level of endotoxin( ET),tumor necrosis factor( TNF),superoxidedismutase( SOD) and malondialdehyde( MDA).(5) Pathological change of colon pre- and post-operation.Results1. Observational results of drainDrain from distal and proximal tube was gas and stool-like liquid after 24hs of operation. And drain continuously increased day by day.2. Mortality after operationNo animal died in test group with fire-arm wound while there is one animal of control died on 3rd,5th,6th,7th days after operation respectively. In group withpuncture wound,no animal died in contrast withl on 3rd and 2 on 6th day after operation in control group.3. Change of pressure in distal and proximal end of stoma after operation.In distal and proximal end of stoma,resting pressures of colon are lower in test group than in control group.4. Pre- and post-operative level of ET,TNF and free radical of oxygenAfter operation,the level of ET and TNF in control group is higher in contrast to the level of them before operation and of test group. The level of MDA in peripheral blood lowered while the level of SOD rised in test group after operation.5. Pathological change of colon pre- and post-operation5.1 Pathological change of colon before operationBefore operation,the wall of colon was swollen and thickened. Observed with optical microsope,cellular atrophy and increased interval between cells were found and WBC infiltrated every layer. With electronic microscope injury of cells,such as space between cells widening and mitochondria disintegrating,can be found.5.2 Pahtological change of colon after operation.5.2.1 Observational results with naked eyesThe colon of animal in test group was empty in distal and proximal end. But in control group,the colon wall was swollen,and it's still enlarged in distal and proximal end. Abscess was found around the stoma in 3 cases.5.2.2 Observational results with microscope.In test group,inflammation around the stoma was slight,and fiber proliferated along the ordinate of colon 7 days after operation. In control group,inflammation around stoma was sever,and degeneration of collagen fiber and focal necrosis of epithelial gland cells of member were found. Proliferation of fiber and inflammation coexisted and disorderly arranged.5.2.3 Observational results with electronic microscope after operationThe interval and linkage between cells as well as the ultrastructure of mitochondria were almost normal in test group. While in control group,it was found that space between cells widened,linkage between cells disappeared and mitochondria in cells disintegrated.ConclusionIn ord...
