Objective Previous studies have shown that prolonged, inhom-ogeneous and anisotropic distribution of connections between myocardial fibers results in the discontinuous anisotropic propagation of sinus impulses.These electrical properties are favoured of the occurrence of atrial fibrillation. Maxium P wave duration(Pmax)> 110ms is a reliable predictor of paroxysmal atrial fibrillation(PAF),The inhomogeneous and discontinuous atrial conduction in patients with PAF has been recently evaluated with a simple electrocardiographic(ECG) index,P wave dispersion(Pd). Pd represents the diferrence between Pmax and minium P wave duration(Pmin).The study is to investigate the relationship between Pd,Pmax and lone PAF deeply,also the effects of Propafenone on Pd,Pmax and Pmin by self-control method. Methods Pd,maxium P wave duration(Pmax) and minium P wave duration(Pmin) were measured in 30 patients with lone PAF and 30 controls as well as in 12 cases effectively treated by propafenone within lone PAF group through self-control study.All 12-lead surface ECGs were obtained at the 50mm/s paper speed and with the gain of 1 rnrn/mv from study participants in supine position during peace respiration. Results Pd and Pmax in lone PAF were significantly greater than those in controls,The sensibility of Pmax =110ms and Pd40ms in AF group were both 83.3%. furthermore, Pd were decreased significantly while the occurence frequency of atrial fibrillation was reduced by propafenone.Conclusion Pd is a reliable index topredict idiopathic PAF and may be used to evaluate drug efficiency.
