Objective: NO is a kind of very unstable low moiecular weight gas. It has many biological character and takes part in the course of and pathology.NO mediates the nerves conduction , takes part in blood vessel relaxated and regulates pancreatic island B cellulae releasing insuline. Perhaps, NO participant the process of onset and development of diabatic peripheral neurepathy (DPN). We used the method of nitratase to determine the serum of NO level in DPN to understand the role of NO in the pathogenesis of DPN and to provide basis for diagnosis of DPN. Methods: All of subjects who were involved were treated at endocrinology department of attached hospital to Liaoning College of TCM. Diabetes mellitus(DM) patient were inaccorded with the diagnostic standard of diabetes stipulated by American Diabetes Association. The diagnostic standard of DPN and diabetic nephropathy(DN) followed the book of Zhong Xueli "Chinical science about diabatic". The subject were divied into 5 groups. There are 20 people in the healthy control group. They are all staff members in attached hospital to Liaoning College of TCM (including retired ones). 18 cases of type 2 DM without complication .51 cases of type 2 DMwith complicatins of DPN. 27 cases of them had nonneuralgia. 24 cases of them had neuralgia. 20 cases of type 2 DM with complication of DN. No difference was confirmed by the test of homoscedasticity in age, sex among each group (p>0.05). By the test of homoscedasticity, no significant difference was found in chinical course of disease between DPN I group and DPN II group. All of the subjects weren't affecting with coronary heart disease ,high blood pressure, cerebrovascular system disease, other nephropathy, heart failure neoplasm and every location of the infection. They have not suffered from diabatic ketosis three weeks rencently. After they had been oral hypoglycemic agents with dietary therapy, the blood glucose of patients kept on the stable level. All of subjects haven't allowed to take vasoactive drug,antioxidant and diuretic or have shutdown taken those durgs for three half-lifes. After an overnight fast , 10ml blood from a cubital vein was taken then centrifuged and stored for determine NO,C-P,TG,TC,LDL-C HDL-C, FBG, HbA1c.At same time , the blood pressure (Bp) at being bedridden in morning. The day before collected blood, examinate the specimen of the objects antisepticurine that has collected for 24 hours for UAER. Mensure superficial peroneal nerve and median nerves sensory nerves conduction velocity, common peroneal nerve motor nerves median nerves conduction velocity by lectromyography of DISA 2500C type. All the datd were expressed as mean+standard deviation, t test was then used for groups with homogeneous variables while t' test for imhomogeneous. Analysis of the NO level of patients that was involved in was adopted by linear regression then by multiple stepwise regression. Results:The serum of NO in DPN groups is significantly lower than that in NC group, SDM group and DN group. The difference between them was significant (p<0.01). NO in DPN I group was lower than that in DPNII group. The two groups show no significant difference (p>0.05). NO in DN group was higher than in DPN groups (p<0.05), lower than SDM group (p<.05). NO significant difference was found between DPN I group and DPNII group in MNCV, SNCV. Analysis by linear regression showed that there was negative correlation between NO level and course of disease Bp TG TC LDL-C FBG HbA1c UAER. The result showed that there was positive correlation between NO level and HDL-C. Analysis by multiple stepwise regression showed that there was significent negative correlation between NO level and FBG course of disease. DiscussionrMost of scholars had found the NO level in diabetic mouse and diabatic patients is lower than in healthy control group at original research and experiment research.
