Objective To investigate the association of angiotensinogen and the gene polymorphism of angiotensinogen(ATG) in the events of the cerebral infarction.Methods Two mutation at +704 from T to C and +521 from C to T in exon 2 of ATG gene were analyzed in 57 patients with ACI,52 patients with lacunae brain infarction,43 essential hypertension and 65 healthy controls by a combination of polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP). The plasma concentrations of angiotensinogen and angiotensin- II (AT- II ) in 57 patients with atherothrombotic cerebral infarction (ACI) and 32 healthy controls were measured by means of radioimmunoassay.Results The plasma ATG concentration in the acute phase of ACI patients (194.47 + 43.83ng/L) was significantly higher than that in the recovery phase of the patients(163.45 + 42.64ng/L) and that in the controls( 159.34 + 45.24ng/L),and that in the severe patients (226.91 + 48.53ng/L) was also markedly higher than that in the mild patients (173.84 + 33.87ng/L). The plasma ATG concentration correlated linearly with the plasma AT- II concentration(r=0.68,p<0.05). The frequencies of 704CC genotype and C allele(54.4% 76.3%) in ACI were significantly higher than that (33.8%,63.8%) in the controls. These frequencies didn't associate with or without hypertension. The plasma ATG concentration with 704CC genotype (196.34 + 44.37 ng/L) were significantly higher than those with 704CT genotype (170.41 +43.64ng/L) (t=2.74,p<0.01) . Compared withthe control subjects the frequency of 704CC genotype and 704C allele in essential hypertension was of significant difference. Between the control group and lacunae brain infarction chi-square test showed no significant difference in the frequencies of 704CC genotype and 23 5C allele. The frequencies of 52ITT genotype and 52IT allele were significantly higher in lacunae brain infarction,not significantly higher in ACI and essential hypertension.Conclusion The event of cerebral infarction can result responsively in the increased concentration of plasma ATG which may be related to the degree of ischemic brain lesions. There was a significant correlation between 704C allele and ACL The 704CC genotype increased risk of ACI. 704C allele may result in the increased plasma AGT which may participant in the development of ACI. 704C allele was the risk factor for essential hypertension. No direct relationship between T704C mutation polymorphism and lacunae brain infarction. There was association between C521T mutation polymorphism and lacunae brain infarction.
