Objective To investigate the effects of cyclosporine (CsA) on the cell proliferation in wound healing following experimental filtration surgery in rabbits eyes. Methods In a prospective randomized control experimental study, 25 New Zealand white rabbits underwent bilateral trabeculectery were divided into 3 different groups. In the first groups , 2% CsA was applied on episclearal for 4 minutes before preparations of a half-thickness scleral flap. In the second gtoups , 0. 2mg/ml MMC was applied on episcleral for 4 minutes, and balanced salt solution was applied in the control group, then the area was irrigated with 100ml balanced salt solution. At 1,4,7,14 and 28 days after surgery , the filtering blebs were observed, the filtering site was examined under light microscope, and the expression of proliferation cell nuclear antigen (PCNA) in the filtering site was also examined with immunohistochemical methods. Results 1. In control group that underwent trabeculectomy but received balanced solution, the number of immunoreactive cells increased 4-7 days after operation and decreased markedly at around 14 days . In the MMC and CsA-treated eyes, immunoreactive cells appeared 4 days after surgery and decreased markedly at around 14 days0 The number of imraunoreative cells in the treated eyes wasmuch lower than that in controls that received balanced salt solution. There was no significant difference between the CsA-treated grounps and the MMC-treated groups. 2. In CsA-treated and MMC-treated eyes, the intraocular pressure ( IOP ) after operation was under 1.93 kPa(14mmHg), the functioning blebs were observed without fibrosis of conjunctive tissue. In control group the effects on the IOP were lower than that of CsA-treated and MMC-treated groups. Conclusion l.CsA suppressed the proliferation of fibroblasts in the filtering site , decreased the IOP after surgery and had effects on the formation of the functioning blebs. There was no significant deffirence between the CsA-treated and the MMC-treated groups, but MMC had much more side effects compared to CsA. 2. CsA have a better effect on inhibiting proliferation and have fewer side effects, thus it is worth of improving its useness.
