Objective To study the relationship between detection of CK20mRNA in the peripheral blood and expression of CK20, Ki67 in gastric carcinoma. Methods The CK20mRNA in the peripheral blood was detected by Reverse Transcription Polymerase Chain Reaction (RT-PCR) from 40 patients with gastric carcinoma before operation, and the expression of CK20, Ki67 in the corresponding tumor tissues was examined immunohistochemistrically by the SP method. Results The positive rate of CK20mRNA in the peripheral blood was 30% (12/40). With the increase of clinical staging, the positive rate of CK20mRNA in the peripheral blood was increased accordingly (X2=20.065,P<0.001); The positive rate of CK20mRNA was inversely proportional to differentiation of adenocarcinoma (X: = 6.541 ,P=0.038). The positive reaction of CK20 was mainly located in cytoplasm and cell membrane, and a small number of nuclei were also stained. The expressive intensity of CK20 was both significantly different in clinical staging groups and adenocarcinoma groups with different grading (X2=32.908 ,1 8.927,P<0.001 ,0.01),the higher the pathological grading of adenocarcinoma was, the stronger theexpression of CK20 was. With the increase of expressive intensity of CK20 in the tumor cells, the positive rate of CK20mRNA in the peripheral blood was increased accordingly (X2=l 1.640,P=0.009). The positive reaction of Ki67 was mainly located in the tumor nuclei, and with the increase of clinical staging or the decrease of pathological grading of adenocarcinoma, the expressive intensity of Ki67 was both increased accordingly (X2=58.467>26.466, P <0.001,respectively). The expressive intensity of Ki67 was directly proportional to that of CK20 (X2=26.083, P=0.002). There was no significant relation between the expression of CK20mRNA, CK20, Ki67 and the histological type (X2=2.773, 8.25 1 , 1.947,P>0.05,respectively). Conclusion The expression of CK20mRNA in the peripheral blood, Ki67 and CK20 in the tumor tissue was all closely related to clinical staging and pathological grading. Because their expression was parallel in gastric carcinoma, and was increased according with the increase of clinical staging and pathological grading, they could be served as indicators for evaluating the metastasis and prognosis on patients.
